下腹部分叶皮瓣在多手指电烧伤中的应用 被引量:3

Repairing electrical burn of multi-fingers by lower abdominal lobulated flaps

摘要 目的探讨下腹部分叶皮瓣修复多手指电烧伤的可行性,并为不相邻多个手指深度创面修复提供临床经验。方法选择1998年至2008年间收治的23例多手指电烧伤患者,创面经手术清创后根据其形状及大小,设计以腹壁浅动脉内外侧支为蒂的下腹部分叶皮瓣修复创面。结果除3例患儿因皮瓣蒂部受压致一叶皮瓣部分坏死外,其余20例患者皮瓣血供丰富,断蒂后创面全部愈合,外形及功能满意。结论腹壁浅动脉解剖结构清楚,血管走向恒定,皮瓣血供丰富,手术操作简单,供区部位隐蔽,是修复多手指电烧伤后深度创面的理想方法。 Objective To study feasibility of repairing simultaneously several fingers by lower abdominal lobulated flaps, and provide experience of recovering multiple deep electrical burn wounds in non-adjacent fingers. Methods Twenty three patients grafted pediculated lower abdominal lobulated flaps after debridement. Then recovering wounds by amputating pedicles of superficial epigastric artery. Results All wounds were healed 1 and had restoration with satisfactory appearance and function but three cases' lobulated flaps were found partial necrosis of pedicle compressed. Conclusions It seems to be an effective way for healing deep electrical burns of multiple fingers by lower abdominal lobulated flaps, which provide constant anatomic structure with superficial epigastric artery, reliable blood supply, less tissue damage, a simple operative route, and secret donor site.
