中西药联合手术Ⅰ期后路固定前路病灶清除植骨治疗腰椎结核8例的体会

摘要 目的探讨抗结核药物与中药联合后路椎弓根内固定前路病灶清除、植骨融合治疗腰椎结核的临床意义。方法2007年1月至2009年3月,收治腰椎(L3~L5)结核患者8例。行强化抗结核药物治疗2-3周后,后路椎弓根螺钉系统撑开矫形、内固定并Ⅰ期前路结核病灶清除、植骨融合手术。围手术期间联合抗结核药物,中药以中药人参养营汤加减以补益气血调服。结果8例均获随访,随访时间8~19个月。X线片示术后7-16个月植骨均骨性融合,后路椎弓根系统内固定位置良好、固定稳定。术后伤口均I期愈合。术后12个月Cobb角为7°-21°,平均15-2°。结核均无复发。结论手术前路病灶术中彻底清除,后路作椎弓根钉系统内固定,有效可靠的脊柱稳定性重建与维护,抗结核药物与中医药结合性治疗确保脊柱结核治愈的关键。
