CT灌注成像对评估原发性骨肿瘤血流灌注的意义 被引量:8

摘要 目的应用64排螺旋CT灌注成像(CTperfusion imaging,CTPI)可以得到体内动态的定量血流参数,但用于骨肿瘤的研究报道很少。文中通过应用CTPI来评价各种原发性骨肿瘤的血流灌注,并进一步探讨各灌注参数的应用价值。方法47例经病理诊断证实为原发性骨肿瘤的患者,治疗前行CTPI检查,获得肿瘤的时间-密度曲线(time-density curve,TDC)和血流量(blood flow,BF)、血容量(blood volume,BV)、达峰时间(time to peak,TTP)、表面通透性(permeability surface,PS)等灌注参数。其中28例为骨肉瘤,11例为软骨肉瘤,8例为骨巨细胞瘤。对3组间灌注参数基础值数据用one-way ANO-VA检验进行统计学分析。结果3组的TDC明显不同,且3组间BF、BV、TTP和PS值的差异均有统计学意义(P均<0.05)。骨肉瘤组与骨巨细胞瘤组的BF、BV及PS值显著高于软骨肉瘤组(P均<0.05),而TTP值显著低于软骨肉瘤组(P均<0.05)。骨肉瘤组的BF、BV及TTP值与骨巨细胞瘤组无显著差异(P>0.05),但PS值显著低于骨巨细胞瘤组(P<0.05)。结论CTPI可以作为一种无创的、有价值的方法来评估原发骨肿瘤的血流灌注。 Objective CT perfusion imaging(CTPI) is a clinical technique that can be used to obtain quantitative measurements of various hemodynamic parameters,but its application to bone tumors has been studied insufficiently.The aim of this study was to investigate the use of CTPI in the evaluation of tumor perfusion in patients with bone tumors.Methods We performed CTPI for 47 patients with primary bone tumor underwent to obtain the time-density curve(TDC) and perfusion parameters including blood flow(BF),blood volume(BV),time to peak(TTP) and permeability surface(PS).Of the total number,28 cases were diagnosed as osteosarcoma,11 as chondrosarcoma,and 8 as bone giant cell tumor(BGCT).The perfusion parameters of the three groups of patients were compared and statistically analyzed by the one-way ANOVA test.Results The three groups showed obvious differences in TDC and significant differences in BF,BV,TTP and PS.BF,BV and PS were significantly higher(P 0.05) while TTP significantly lower(P 0.05) in the osteosarcoma and BGCT than in the chondrosarcoma patients,with no significant differences in the BF,BV and TTP value between the osteosarcoma and BGCT groups(P 0.05).PS was significantly lower in the osteosarcoma than in the BGCT group.Conclusion CTPI is a valuable noninvasive method for evaluating the perfusion of bone tumor.
