复合振动仪治疗骨质疏松对骨代谢影响6个月临床观察 被引量:4

摘要 目的观察复合振动仪治疗骨质疏松(osteoporosis,OP)对骨代谢的影响。方法70例骨质疏松志愿者随机分成复合振动组(whole body compound vibration training group)35例和对照组(control group)35例,复合振动组采用自行研制的复合振动仪进行全身振动治疗,振动频率45~55Hz,振动强度0.5~0.8g,30min/次,1次/隔天,连续6个月,对照组不进行任何干预,于试验第0、1、3、6个月分别检测复合振动组和对照组碱性磷酸酶(alkaline phosphatase,ALP)、骨钙素(osteocalcin,OC)、抗酒石酸酸性磷酸酶(tartrate-resistance acid phosphatase,TRAP)、钙磷乘积(calcium-phosphorus product,[Ca]×[P])的变化。结果第3、6个月,ALP、OC、TRAP、[Ca]×[P]的P分别为0.003、0.686;0.768、0.618;0.817、0.855;0.077、0.001。ALP早期呈上升的趋势,到第3个月形成一个峰值,然后呈下降趋势,[Ca]×[P]呈不断上升的趋势,OC、TRAP无明显变化趋势。结论复合振动可促进骨组织钙化、促进钙和磷以骨盐形式沉积于骨组织;连续治疗可能使人体产生耐受,降低复合振动的治疗效应,采取短时、间断的加载方式可能更有利于促进复合振动的成骨效果。 Objective To observe the effect of bone metabolism of compound vibration machine on the treatment of osteoporosis. Methods Seventy volunteers of osteoporosis were selected from a gcrocomium and were randomly assigned to a whole body compound vibration training group ( WBCV, n-35) and a control group(CON, n-35). The WBCV group intervention consisted of a 6-month whole body compound vibration (30 rain/day, 1 time/qod). During the 30-minute vibration program, the platform oscillated in an ascending order from 45 to 55 Hz, corresponding to estimated maximum compound accelerations from 0.5 g to 0. 8 g. The CON group did not participate in any training, just keeping the normal life as before. Alkaline phosphatase (ALP) , osteocalcin ( OC ) , tartrate-resistance acid phosphatase (TRAP) , calcium-phosphorus product were measured after the 0, 1, 3,6-month intervention. Results At the 3rd month and the 6th month,the P of alkaline phosphatase, osteocalcin, tartrate-resistance acid phosphatase, calcium-phosphorus produc are respectively 0. 003,0. 686 ;0. 768,0. 618 ;0. 817,0. 855 ;0. 077,0. 001. ALP increase early, to a peak in the 3rd month, then is into the downward trend. [ Ca]× [ P] is in the ever-increasing trend. OC and TRAP have no significant change in trend. Conclusions Composite vibration may promote bone tissue calcification, may promote calcium and phosphorus deposition in bone tissue in the form of bone salt. The effect of compound vibration on the treatment of osteoporosis may reduce if doing continuously, and to take short-term, intermittent load may be more conducive to the promotion of osteogenic effect of composite vibration.
