摘要
目的探讨老年鼻饲患者每次鼻饲量及间隔时间。以确立科学的、适合老年人生理的鼻饲量值。方法按纳入标准选择长期鼻饲老年患者180例,按年龄段,61～70岁患者随机分为实验1、实验2和对照组1;71～80岁患者分为实验组3和实验组4和对照组2。每组30例。在鼻饲置管方法、鼻饲体位、鼻饲液成份、符合营养热卡的前提下,对照组按常规方法进行鼻饲;实验组按照实验方法鼻饲,6组均观察餐后返流、胃潴留、腹胀、腹泻、便秘、末次饲食后6h微量血糖值、血清白蛋白、总蛋白、体重情况。结果6组老年鼻饲患者在不同鼻饲量及间隔时间下其鼻饲并发症比较差异无显著意义;营养状况:白蛋白及总蛋白比较差异有显著意义。结论实验组鼻饲法的每次鼻饲量和间隔时间是安全的、符合老年人生理的鼻饲量值。
Objective To study the single feeding volume and time interval for elderly patients with nasal feeding, and to establish a scientific, physiological nasal feeding value suit for the elderly. Method According to inclusion criteria, 180 cases of elderly patients with long-term tube feeding were selected. Patients aged 61 to 70 years old were randomly divided into experimental group 1, experimental group 2 and the control group 1. Patients aged 71 to 80 years old were randomly divided into experimental group 3, experimental groups 4 and control group 2. There was 30 cases in each group. The tube method, posture and liquid composition of nasal feeding were in line with the body＇s nutrition and calories needs. The control groups were used conventional nasal feeding, while experimental groups were given experimental nasal feeding. The postprandial reflux, gastric retention, abdominal distension, diarrhea, constipation, blood glucose values of 6 hours after feeding, serum albumin, total protein and weight change for 6 groups were observed and statistical analysis.Result There was no significant different in complications among 6 group with different feeding volume and intervals. But comparison of nutritional status showed that serum albumin and total protein were statistically significant. The serum albumin and total protein for patients in experimental group was higher than that of the control group. The experimental method reduced significantly the workload of day care and extended the time to rest for nasal feeding patients.Conclusion Nasal feeding volume and time interval in each experimental group are safe, and accord with the elderly patients＇ physiological nasal feeding.
