摘要
【目的】探讨青少年肥胖与生长激素释放肽(Ghrelin)的关系。【方法】158名13~15岁青少年,根据体重指数(BMI)分为对照组80例,超重组44例和肥胖组34例。测量身高、体重及腰围(WC)。空腹静脉采血,应用酶联免疫吸附法检测血清真胰岛素(TI)和Ghrelin水平。【结果】①超重组BMI、WC和空腹血糖(FPG)高于对照组(P〈0.05),肥胖组BMI和wc高于超重组(P〈0.05),超重组和肥胖组TI和胰岛素抵抗指数(HOMA-IR)均高于对照组(P〈0.01),三组间Ghrelin水平比较差异无显著性(P〉0.05)。②Ghre-1in与FPG和ln(HOM站IR)存在单变量相关。多元线性回归显示以Ghrelin为因变量,在校正年龄、性别和BMI后,FPG和ln(HOMA—IR)为独立危险因素。③三组分别进行多元线性回归分析显示,对照组FPG、超重组ln(HOMA-IR)和FPG、肥胖组ln(HOMA—IR)分别为Ghrelin独立危险因素。【结论】FPG水平和胰岛素抵抗(insulin resistance,IR)是导致青少年血Ghrelin水平变化的独立危险因素,在肥胖青少年IR,而非FPG,可能是Ghrelin水平变化的独立预测因素。。
[Objective] To explore the relationship between obesity and ghrelin in adolescent. [Methods]A total of 158 adolescents aged 13-15 years were divided into control group( n = 80), overweight group( n = 44) and obesity group( n = 34) according to body mass index(BMI). Height, weight and waist circumference (WC) were measured. Blood samples were collected. Serum true insulin(TI) and ghrelin were assayed by ELISA. [Results] Overweight group had significantly higher levels of BMI, WC and fasting plasma glucose (FPG) than control group ( P 〈0.05). Obesity group had significantly higher levels of BMI and WC than overweight group ( P 〈0.05). Overweight and obesity group had significantly higher levels of TI and insulin resistance index (HOMA-IR) than control group( P 〈0.01). Ghrelin levels among three groups showed no significant difference( P 〉0.05). Ghrelin was positive correlation with FPG and ln(HOMA-IR). Multiple linear regression analysis using ghrelin as the dependent variable showed that FPG and ln(HOMA-iR) were sig- nificant independent predictors for ghrelin adjusted for age, sex, BMI and blood pressure. Multiple linear regression analysis in three group respectively showed that FPG in control group, ln(HOMA-IR) and FPG in overweight group and ln(HOMA-IR) in obesity group were significant independent predictors for ghrelin, respectively. [Conclusion]FPG levels and IR are the independent risk factors of blood ghrelin levels in adoles- cent. IR, but not FPG levels, may be the independent predisposed factor for ghrelin levels in obese adolescents.
