摘要
目的探讨乳腺癌患者凋亡抑制蛋白Survivin表达与血清糖类抗原153(CA153)检测的临床意义及二者的关系。方法分别应用免疫组织化学法和化学发光免疫法检测65例乳腺癌中Survivin表达情况与血清CA153的水平,采用SPSS13.0统计软件,Survivin表达与临床病理学指标的关系用χ2检验,Survivin和CA153的关系采用Sperman等级相关性分析。结果乳腺癌患者治疗前血清CA153浓度为(35.6±11.3)U/ml,治疗后为(11.8±3.6)U/ml,差异有统计学意义(t=2.915,P<0.05);乳腺癌Survivin阳性率在不同肿瘤大小、组织学分级和临床分期中差异无统计学意义(P>0.05);Survivin阳性表达患者中腋窝淋巴结转移率为68.0%(24/50),Survivin阴性表达患者腋窝淋巴结转移率为6.7%(1/15),差异有统计学意义(χ2=17.465,P<0.001);乳腺癌患者CA153浓度越高,Survivin表达阳性强度越大,二者呈正相关(r=0.425,P<0.05);Survivin表达阳性强度越大,腋窝淋巴结转移率越高。结论乳腺癌中Survivin、CA153的联合检测可以判断预后,二者升高明显提示患者可能有腋窝淋巴结转移,预示患者预后不良。
Objective To investigate the expression of survivin protein and the level of serum CA153 in patients with breast carcinoma. Methods The expression of survivin and the level of serum CA153 were examined respectively by immunnhistoohemical method and chemiluminescent immunoassay in 65 patients with breast cancer. Correlation between survivin expression and other pathological variables of breast cancer was determined by Chi-square test, and the Spearman rank correlation test was used for analysis of survivin and CA153. Results In breast cancer patients, the level of serum CA153 was significandy higher before treatment than after treatment ( t = 2. 915, P 〈 0. 05 ). The positive expression of survivin was not statistically different by tumor size, histopathologie grade and clinical stage (Ps 〉 0. 05 ). The incidence of lymph node metastasis in patients with survivin expression was significantly higher than that in those without expression (68.0 % vs 6. 7 %, X^2 = 17. 465, P 〈 0. 001 ). A positive correlation was noted between the levels of CA153 and the expression levels of survivin ( r = 0. 425, P 〈 0. 05 ), and between survivin expres- sion and the incidence of lymph node metastasis (r = O. 546, P 〈 0. 001 ). Conclusion Prognosis may be determined by combined assays of CA153 and survivin. The elevation of the two markers may suggest higher incidence of lymph node metastasis, and thus poor prognosis.
