摘要
室间隔缺损(VSD)解剖复杂,常分为膜周型、双动脉下型、肌部型。介入封堵术中,膜周部型的易影响到希氏束,双动脉下型的易影响到主动脉瓣。其手术难度大、并发症多且严重,特别是嵴内型VSD、干下型VSD、巨大VSD伴肺动脉高压、隔瓣下型VSD等。近年来,随着器械、技术等的不断改进、提高,手术成功率明显提高,并发症明显降低。而Hybrid技术的践行,使复杂先天性心脏病的介入治疗近年来也取得了突破。
The access to ventricular septal defect(VSD)is anatomically difficult.VSD is usually divided into perimembranous,doubly committed subarterial and muscular VSD.In the transcatheter closure of VSD,the His bundles are subject to the interference from the interventions on the perimembranous VSD while the aortic valves are subject to that from the interventions on the doubly committed subarterial VSD.The treatment of VSD is technically challenging with frequent and serious complications,especially for intracristal VSD,subconal VSD and huge VSD with pulmonary hypertension,and septal-leaflet VSD.Recently,with the improvement in equipments and technologies,the success rate has been increased while the frequency of complications has been reduced.Additionally,the application of Hybrid technology results in some breakthrough in the interventional treatment of complicated VSD.
