应用心肌速度分布图评价冠心病左室局部收缩功能的研究 被引量:1

摘要 目的 探讨组织多普勒心肌速度分布图((myocardial velocity profile,MVP)评价冠心病患者左室短轴方向局部收缩功能的应用价值。方法选择43例经冠状动脉脉造影证实为前降支狭窄的急性心肌梗死患者作为冠心病组,35例健康体检者作为对照组,测定左室乳头肌水平前壁、前间隔、下壁、后壁短轴方向的心肌运动速度,得出MVP和心肌运动速度梯度((myocardial velocity gradient,MVG)。结果冠心病组前壁及前间隔收缩期MVG均显著低于对照组,分别为【(0.9±0.4)s-1对(4.8±0.5)s-1,P〈0.01及(1.0+0.5)s。对(5.4±0.7)s,P〈0.01】;下壁及后壁MVG无明显差异,分别为【(4.2±0.7)s。对(4.9±0.6)s^-1,P〈0.01及(4.64±0.8)s^-1对(5.0±0.5)s^-1,P〈0.01】。结论MVP能准确地反映冠心病患者心梗区域心肌短轴方向上的运动速度变化.提供了一种评价冠左室短轴方向局部收缩功能的新方法。 Objective To evaluate the regional left ventricle systolic function in short-axis orientation in patients with coronary heart disease (CAD) by myocardial velocity profile (MVP). Methods 43 cases with acute myocardial infarction which had stenosis in anterior descending branch conformed by coronary angiography were enrolled as CAD group, 35 healthy volunteers were selected as control group. Transmural wall-motion velocities of anterior wall (AW), anterior septal (AS), inferior wall (IW) and posterior wall (PW) were recorded, then MVP and myocardial velocity gradients (MVG) were calculated. Results There was a significantly difference between two groups in MVG at AW and AS, [(0.9±0.4)s^1vs (4.8±0.5)s^-1,P〈0.01 and (1.0±0.5)s-1 vs (5.4±0.7)s-l,P〈0.01, respectively]. Conclusion The regional left ventricular systolic function in short-axis orientation was decreased in patients with CAD. The MVP and MVG may be a new method for evaluating regional left ventricular contractile abnormalities.
