摘要
目的探讨血清可溶性细胞表面分化抗原CD40配体(sCD40L)与2型糖尿病血管并发症的关系及临床意义。方法采用酶联免疫吸附法检测58例并发急性冠脉综合征或急性脑梗死大血管并发症的2型糖尿病患者及61例无大血管并发症的2型糖尿病患者血清sCD 40L浓度,并以57例健康人作为对照。空腹血糖及血脂按常规方法测定。结果①2型糖尿病并大血管并发症患者血清sCD40L浓度明显高于无大血管并发症患者和健康对照组(P(0.01);无大血管并发症患者血清sCD 40L浓度高于健康对照组(P(0.05)。②2型糖尿病并发急性冠脉综合征与并发急性脑梗死各亚组血清sCD 40L浓度均显著高于无大血管并发症组和健康对照组(P(0.05);而上述两个亚组间的血清sCD 40L浓度无显著差异(P(0.05)。③2型糖尿病大血管并发症组血清sCD 40L浓度与低密度脂蛋白(LDL-C)水平呈正相关(r=0.299,P(0.05);2型糖尿病无大血管并发症组血清sCD 40L浓度与LDL-C、TG水平呈正相关(r=0.310,P(0.05;r=0.236,P(0.05)。结论血清sCD 40L可能参与了2型糖尿病大血管并发症的发病过程,并可作为判断2型糖尿病患者有无大血管并发症的一项指标。
Objective To explore the relation between serum sCD 40L level and type ;2 diabetic macrovascular complications. Methods Serum levels of sCD 40L was measured with Enzyme - Linked immuosorbent assay in 58 cases of type 2 diabetic patients with macrovascular complications, 61 cases of the absence of any major vascular complications in patients with type 2 diabetes and 57 healthy subjects as controls. Results ① Serum levels of sCD 40L of type 2 diabetic patients with macrovascular complications were significantly higher than those without maerovascular complication and control (P 〈 0.01 ). sCD 40L levels of type 2 diabetic patients without maerovaseular complications were significantly higher than control(P 〈 0.05 ).② Serum levels of sCD 40L of type 2 diabetic patients with acute coronary syndrome and acute cerebral infarction were significantly higher than those without macrovascular complication and control(P 〈 0.05 ). Serum levels of sCD 40L of type 2 diabetic patients with acute coronary syndrome were no significantly than those with acute cerebral infarction (P 〉 0.05 ). ③ Serum levels of sCD 40L in type 2 diabetic patients with maerovaseular complications had positive relative with LDL-C (r = 0.299,P 〈 0.05). Serum levels of sCD 40L in type 2 diabetic patients without maerovascular complications had positive relative with HDL- C and triglyeerides ( r = 0. 310, P 〈 0.05 ), (r = 0. 236,P 〈 0.05). Conclusion Serum sCD 40L may be involved in the pathogenosis of type 2 diabetes process, which can serve as a index to judge maerovaseular complications in type 2 diabetic patients.
