万古霉素硫酸钙与万古霉素骨水泥在体内抗生素释放规律的动物实验观察比较 被引量:8

摘要 目的与万古霉素骨水泥相比较,探讨万古霉素硫酸钙在体内抗生素释放的规律。方法将成功建立左胫骨慢性骨髓炎的16只健康成年家兔随机分成两组:实验组(万古霉素硫酸钙组)和对照组(万古霉素骨水泥组),在左胫骨分别植入万古霉素骨水泥栓及万古霉素硫酸钙栓,收集两组实验对象术后0.5h、1h、1.5h、2h、4h、6h、12h、24h、48h、72h的引流液及血液,测量万古霉素药物浓度至引流液消失、血液中药物浓度测不到为止;收集1h、2h、6h、12h、24h、48h、4d、7d的尿液测量药物浓度,至尿液药物浓度消失并比较两组的药物浓度的差异。结果在观察时间范围内,实验组引流液在各个时间点的药物浓度明显高于对照组,差异均有统计学意义(t分别=3.25、3.34、3.24、3.75、3.96、4.56、5.32、5.53,P均<0.05)。对照组血液药物浓度和尿液药物浓度一直处于测不到的状态,而实验组血液药物浓度在植入后1.5h开始被检测到药物浓度,与对照组同时间点药物浓度比较,差异均有统计学意义(t分别=2.32、3.01、2.54、2.33、1.95、2.17,P均<0.05);实验组植入后1h开始测到尿液药物浓度,药物浓度迅速增高,于24h达到高峰,于1周后下降至极低的药物浓度,其中2~48h与对照组同时间点尿液药物浓度比较,差异均有统计学意义(t分别=2.55、3.22、3.48、4.13、3.46、3.15,P均<0.05)。结果万古霉素硫酸钙在体内的抗生素释放优于万古霉素骨水泥。 Objective To investigate the pattern of antibiotic release of vancomycin-calcium sulfate in rabbits with chronic staphylococcal osteomyelitis, and compared with vancomycin-polymethylmethacrylate (PMMA). Methods Totally 16 adult rabbits with chronic osteomyelitis in left tibiae were randomly divided into two groups and implanted with vancomycin-calcium sulfate (experimental group) and vancomycin-polymethylmethacrylate (control group) respectively. Drainage fluid and blood were collected at 0.5 hours,1 hour,l.5 hours,2 hours,4 hours,6 hours,12 hours,24 hours,48 hours after operation. Urine were also collected 1 hour,2 hours,6 hours, 12 hours,24 hours,48 hours,4 days, 7 days after operation. The concentrations of vancomycin in each sample were measured. Results The vaneomycin concentrations of drainage fluid were significantly higher at each time point in experimental group, the differences were statistically significant (t=3.25,3.34,3.24,3.75,3.96,4.56,5.32,5.53,P〈0.05). In experimental group, the vancomycin concentrations were detected 1.5 hours after implantation in blood. However, in control group, the vancomycin concentrations were undetectable in blood and urine all the time. There were significant differences at same time point (t=2.32,3.01,2.54,2.33,1.95,2.17,P〈0.05).The vancomycin concentrations were detected l hour after implantation in experimental group. The drug concentration increased rapidly, peaked at 24 hours, and dropped to very low after 1 week. Compared with the control group, differences were statistically significant during 2 hours to 4 days (t=2.55,3.22,3.48,4.13,3.46,3.15,P〈0.05). Conclusions Vancomycin release of vancomycin-calcium sulfate is better than that of vancomycin-polymethylmethacrylate.
