摘要 目的:研究血管内皮生长因子(VEGF)表达与骨肉瘤血管生成和预后的关系。方法:应用免疫组化和形态计量方法,检测80例骨肉瘤VEGF表达和肿瘤微血管密度。结果:骨肉瘤VEGF表达(-)级者6例,(±)级者10例,(+)级者22例,(++)级者42例。VEGF表达与肿瘤体积、Dahlin's分型和Price's分级无关,但与肿瘤微血管密度计数相关(P<005)。VEGF高表达组(++)级多见于骨样组织幼稚型(P<001),而且其生存率明显低于VEGF低表达组。结论:VEGF是骨肉瘤重要的血管生成因子,其表达可能是估价骨肉瘤预后的一项有价值的指标。 ims: To investigate the relationship between vascular endothelial growth factor(VEGF)and angiogenesis as well as its prognostic signifiance in osteosarcoma. Methods: The expresson of VEGF and microvascular density count in 80 osteosarcomas were investigated by immunohistochemical staining and morphometry. Results: Out of 80 cases osteosarcomas,6 cases were graded as(-),10 cases as(±),22cases as(+)and 42 cases as(++)of VEGF expression.The high VEGF expression rate was not significantly different with respect to tumor size,Dahlin's classification and price's grading,but it was directly correlated with the extent of neovascularization,also the high VEGF expression rate was much lower in the group of well-differentiated tumor osteoid type than in the group of poor-differentiated tumor osteoid type.In addition,postoperative survey of 80 patients who had been followed up for at least 5 years demonstrated that the survival rate of patients with high VEGF-expression tumors was significantly lower than that of patients with low VEGF expression tumors(P<005). Conclusions: These findings support the hypothesis that VEGF is an important angiogenic factor in osteosarcoma. VEGF expression may be a useful prognostic indicator of osteosarcomas.
