区域动脉灌注乌司他丁治疗犬急性出血坏死性胰腺炎的机制 被引量:8

摘要 目的:探讨区域动脉灌注乌司他丁(UTI)治疗犬急性出血坏死性胰腺炎(AHNP)的机制。方法:制备犬AHNP模型,随机分组。A组胃十二指肠动脉灌注生理盐水(NS),B组外周静脉注入UTI+NS,C组胃十二指肠动脉灌注UTI+NS。在不同时间段抽血,测血中淀粉酶、丙二醛(MDA)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)含量。测胰腺组织TNF-αmRNA和IL-6mRNA的表达。结果:3组模型建立后血清淀粉酶、MDA水平明显上升。B组、C组与同时间点A组相比,淀粉酶、MDA水平明显下降,其中以C组下降最明显。3组模型建立后血中TNF-α、IL-6蛋白水平均明显上升,与同时间点A组相比,B组只在部分时间段下降,而C组在各时间段均下降。B、C组胰腺组织TNF-αmRNA和IL-6mRNA的表达明显下调,以C组下调最明显。结论:区域动脉灌注UTI治疗犬AHNP的机制可能有:(1)使UTI早期在胰腺组织中就达到较高的浓度,且维持时间更长;(2)抑制胰酶的分泌;(3)清除氧自由基;(4)下调胰腺组织TNF-αmRNA和IL-6mRNA的表达,降低血中TNF-α和IL-6蛋白的水平,阻断促炎因子的瀑布样级联放大效应。 Objective To explore the mechanism of regional intraarterial infusion of ulinastatin (UTI) in the treatment of acute hemorrhagic necotizing pancreatitis (AHNP). Methods A canine model of AHNP was established and the dogs were then randomized to group A (infusion of NS via gastropiploic artery), group B (UTI and NS via peripheral vein), or group C (UTI and NS via gastropiploic artery). Serum levels of amylase, MDA, TNF-α, and IL-6 were determined at different time points. Expressions of TNF-α mRNA and IL-6 mRNA in the pancreatic tissues were detected. Results The levels of serum amylase and MDA were markedly elevated in the dogs with AHNP. Levels of amylase and MDA were lower in groups B and C than in group A, with a notable reduction in group C. The levels of TNF-α and IL-6 significantly increased in the model of AHNP, Compared with group A at the same time point, the levels of TNF-α and IL-6 reduced in group B at some time points, but decreased in group C at every time point. The expressions of TNF-α mRNA and IL-6 mRNA were evidently downregulated in groups B and C, with a more obvious change in group C. Conclusions The mechanisms of action of UTI can be that UTI inhibits the release of pancreatic enzymes, eliminates free oxygen radicals, downregulates expressions of TNF-α mRNA and IL-6 mRNA in the pancreatic tissue, reduces serum levels of TNF-α and IL-6, and blocks the effect of waterfall-like enlargement of pro-inflammatory cytokines ; while it reaches a higher level in the pancreatic tissue in a early stage.
