彩超对高危妊娠胎儿脐动脉及大脑中动脉血流动力学变化的研究 被引量:2

摘要 摘 要:目的 探讨高危妊娠胎儿脐动脉(UA)和大脑中动脉(MCA)的血流动力学变化规律,评价彩色多普勒超声在预测围产儿结局中的临床应用价值.方法 应用彩色多普勒超声对168例高危妊娠胎儿的脐动脉及大脑中动脉的血流进行检测,分别测量以下血流参数:收缩期峰值速度(S)、舒张期末速度(D)、平均速度(MEAN)、收缩期峰值速度与舒张期末速度之比值(S/D)、阻力指数(RI)、搏动指数(PI),计算出MCAPI/UAPI的比值,并与正常妊娠组116例作对照分析.结果 高危妊娠组胎儿脐动脉血流S/D,RI,PI值高于正常妊娠组(P〈0.05),而胎儿大脑中动脉血流S/D,RI,PI值低于正常妊娠组(P〈0.05),MCAPI/UAPI比值低于正常妊娠组(P〈0.05);正常妊娠组MCA和 UA的S/D,RI,PI呈直线正相关(P〈0.01),而高危妊娠组MCA和 UA的S/D呈负相关(P〈0.01),RI,PI无相关关系(P〉0.05);高危妊娠组围产儿结局异常的百分比明显高于正常妊娠组(P〈0.01).结论 高危妊娠胎儿脐动脉及大脑中动脉的血流动力学超声参数呈反相变化,二者联合检测对预测高危妊娠胎儿宫内缺氧及不良结局有重要临床价值. Objective To investigate hemodynamic changes in umbilical and middle cerebral artery ( MCA ) of high risk pregnant fetuses and to evaluate the clinical application value of color Doppler uhrasonography in predicting perinatal outcome. Methods Color Doppler uhrasoud was used to examine the following blood flow paramete of fetal MCA and umbilical artery( UA) in high risk pregnancy group( 168 cases) and normal pregnancy group( 116 cases) :the systolic peak velocity( S) ,the end-diastolic flow velocity( D) ,the mean velocity(MEAN ) ,systolic/diastolic ratio( S/D ) ,resistance index( RI ) and pulsatility index( PI ) of MCA and UA. MCAPI/UAPI ratio was calculated. Results Compared with normal pregnant group, the S/D, RI and PI of UA in high risk pregnant fetuses increased significantly, while the S/D, RI and PI of MCA decreased significantly and MCAPI/UAPI ratio decreased significantly ( P 〈 0.05 ). In normal pregnancy group, there was positive correlation between S/D, RI, PI of MCA and those of UA, while in high risk pregnancy group, there was negative correlation between S/D of MCA and that of UA and there was no statistical correlation between RI,P1 of MCA and those of UA. The adverse perinatal outcome incidences in the high risk pregnant group were significant higher than that of normal pregnant group. Conclusion The hemodynamic changes regularity of UA and MCA was in opposite way in high risk pregnant fetuses. Detecting UA and MCA at the same time was valuable for predicting fetal intrauterine hypoxia and adverse perinatal outcome of high risk pregnant fetuses.
