The effects of acute experimental hyperglycemia on cardiac function of rabbits

摘要 目的探讨1型糖尿病动物模型短期高糖血症对心脏功能的影响。方法采用四氧嘧啶致1型糖尿病兔模型为糖尿病(DM)组,并设正常对照组(Con组)。分别在成模后2周和4周,使用心脏多普勒超声和左室导管检查评价心脏功能,并行病理学检测。结果与Con组比较,DM 2周组(DM 2W)心功能指标无明显变化;DM4周组(DM 4W)超声检测示E/A倒置,舒张早期充盈减速时间延长,左室射血分数无明显改变。左室导管检查示左室舒张末压升高(7.55±0.31 vs 3.32±0.34mmHg,DM 4W vs Con,P<0.05),松弛时间常数延长(21.92±4.65 vs 12.64±3.21ms,DM 4W vs Con,P<0.05),左室压力最大下降速率降低。结论 1型糖尿病兔模型4周高糖血症可导致心脏出现明显的舒张功能减退,收缩功能无下降。 Objective To clarify the features of diabetes mellitus-induced rabbit cardiac dysfunction in a short-term in rabbit model of type 1 diabetic mellitus, and to investigate the effects of acute experimental hyperglycemia on cardiac function of rabbits. Methods Diabetes was induced by alloxan injection. After 2 and 4 weeks, noninvasive echocardiography and invasive left ventricular catheterization at rest were performed in diabetic (DM 2W, n=8; DM 4W, n= 7) and control rabbits (Con, n= 16). After catheterization, rabbits were killed for histological studies and blood samples were taken for the measurement of biological characteristics. Results No significant differences were found in eehocardiography and cardiac catheterization parameters between control and diabetic rabbits for 2 weeks. In 4 weeks group, echocardiography revealed that impaired LV diastolic function expressed by diminished E-waves (45.41±8. 68 vs 60.66±7.89 cm/s in DM 4W vs Con, P〈0.05), increased A-waves(64.32± 6.59 vs 33.91 ±4.92cm/s in DM 4W vs Con, P〈 0. 05), reverted E/A ratio and increased E-wave deceleration time. Concurrently, through cardiac catheterization, LV end-diastolic pressure (7. 55±0. 31 vs 3.32±0. 34 mmHg in DM 4W vs Con, P〈0.05)and diastolic time constant (21.92±4.65 vs 12.64± 3.21ms, DM 4W vs Con, P〈0.05)were increased, and minimum dp/dt was reduced. There were no significant differences in LV ejection fraction, LV peak systolic pressure, or maximum dp/dt. Conclusions Results suggest that acute hyperglycemia for 4 weeks can induce left ventricular diastolic dysfunction eharaeterized by both impaired active relaxation and increased passive chamber stiffness. Whereas, left ventricular systolic function can remain normal.
