摘要
目的通过应用二维斑点追踪显像技术(2DSTI)和定量分析软件对腹主动脉瘤(AAA)径向运动进行观测,与正常组进行比较,探讨该技术对腹主动脉瘤局部运动异常的研究价值。方法首先对30名正常人(正常组)腹主动脉分别采用M型超声和2DSTI进行前后壁位移测量。然后采用2DSTI对30名正常人、AAA患者(AAA组)21例进行管壁局部测量。采集3个心动周期的图像,进入Qlab脱机软件分析,取两组相应各节段各心动周期收缩期径向最大位移及舒张期径向最大速度的平均值。结果①正常组腹主动脉收缩期前、后壁位移M型超声测量与2DSTI测量结果相比无明显差别(P>0.05);②正常组收缩期最大径向位移前壁位移大于其余各壁(P<0.05),其余各壁之间比较无明显差异,舒张期最大径向速度各节段之间比较无明显差别(P>0.05);③AAA组各节段收缩期最大径向位移明显小于正常组(P<0.05),舒张期最大径向速度小于正常组各节段(P<0.05);④彩色多普勒显示正常组腹主动脉内呈现单一的红色层流,AAA组出现涡流及旋流,血流信号紊乱。结论 2DSTI可检测正常腹主动脉管壁及腹主动脉瘤壁径向运动,能够反映AAA局部各节段运动位移及速度的异常。
Objective By using two-dimensional speckle tracking imaging(2DSTI) and quantitative analysis software to observe abdominal aortic aneurysm(AAA) radial movement and compared the AAA group so as with the normal group to find the value of the technology on the study of AAA local abnormal motion.Methods Normal abdominal aorta anterior and posterior wall displacements of normal group were measured by M-mode ultrasonography and 2DSTI,then using 2DSTI for partial measurements of two groups;three cardiac cycle images and import them into Qlab off-line software were collected.Average values of systolic radial maximum displacement and diastolic radial maximum speed of the corresponding segment during three cardiac cycles were calculated.Results 1) normal abdominal aortic systolic anterior and posterior displacements measured by M-mode ultrasound and 2DSTI had no significant difference measurements(P 0.05);2) normal group systolic radial maximum displacements of the anterior segment were larger than the other segments(P0.05),the rest segments showed no significant difference between them(P0.05),diastolic maximum radial velocity of all segments showed no significant difference(P0.05);3) systolic radial maximum displacement of AAA group was smaller than that of the normal group of all segments(P 0.05) and diastolic maximum radial velocity was less than the normal group of all segments(P0.05).4) Color Doppler showed it was laminar flow in normal abdominal aorta and in AAA were swirling vortex flow.In AAA,the blood flow signal appeared disturbance.Conclusion 2DSTI can detect the radial movement of normal abdominal aorta and AAA to reflect the anomalies of AAA local segmental displacement and velocity.
