摘要
目的总结分析经直肠超声引导下困难性男性导尿技术。方法本院在常规导尿术操作过程中出现导尿困难的老年男性患者共l8例。将超声高频探头伸入直肠内,超滑导丝及导管徐徐插入尿道,在超声引导下,用导丝小心探查通过尿道狭窄处,一旦导丝通过狭窄处,就很容易进入膀胱。结果 18例困难性男性导尿病例在经直肠超声的引导下均成功完成导尿,尿液引流通畅。操作过程平均历时18.2 min。患者无明显疼痛,无尿路感染,无导丝、导管穿破尿道等并发症出现。尿管拔出后排尿困难与导尿前比较无变化。结论经直肠超声引导导尿创伤小,避免了常规导尿时的盲目反复插管操作,具有在临床上推广使用的价值。
Objective To summarize the techniques of transrectal ultrasound-guided catheterization of the male patients with the failure of catheter insertion. Methods There were 18 aged male patients with the failure of conventional catheter insertion in our hospital. High frequency ultrasound probe be put into the rectum. The guidewire and catheter were slowly inserted into the urethra, and passed through the stenosis of urethra carefully with the ultrasound guide. Once the guidewire passed through the stricture,it was easy for the guidewire to enter into the bladder. Results Ultrasound guided catheterization of all the 18 patients was successfully conducted. Urine drainage was unobstructed. The average time of the operation was 18.2 minutes. No obvious pain emerged. No complications,such as urinary tract infection and perforation of the urethra by guidewire and catheter,occurred. Compared to the pre-catheterization,dysuria unchanged after the catheter pulled out. Conclusion Ultrasound-guided catheterization is a minimal invative method. It could also avoid the conventional catheterization of blindly repeated intubation,and is worthy to be widely used in the clinical activities.
