摘要
目的了解城市社区糖尿病患者管理的现状,存在的问题及对策。方法对社区卫生服务中心9名分管主任个别访谈,对110名相关工作人员问卷调查。结果闸北区社区糖尿病管理有87.5%的社区采用"条块结合"模式。在工作人员中,46岁以上者占48.2%,中专学历者占50.0%,初级职称者占56.4%。62.7%认为访视下沉到站点,医患熟悉程度"明显加强"或"加强"。访视人员"大部分能"和"完全能"提供"临床治疗、血糖监测、用药"和"个性化膳食运动"健康指导比例分别占59.8%和57.6%,77.2%的人能"及时分析和反馈患者病情控制情况"。站点临床医护人员上述能力均显著高于公卫人员(P<0.05)。35.8%认为质控"对随访技巧和工作细节提出了改进建议",26.9%和4.6%认为质控指标客观性"较好"和"很好"。3.6%和27.3%的人认为患者依从性"很好"和"较好",21.8%的人认为患者对随访"欢迎"和"很欢迎"。结论闸北区糖尿病社区管理"条块结合"模式体现出工作效率提高、医患关系改善、全科服务推进和质控工作加强等优势;同时还存在实际知识技能不能满足需要,全科医生"重医轻防",糖尿病患者依从性欠佳,缺乏客观、规范的质控标准等问题。
OBJECTIVE To know the current situation, problems and counter measures of diabetes management in urban communities. METHODS Individual interviews was used to nine vice directors of community health service centers, and questionnaire survey was used to 110 related staffs. RESULTS There were 87.5% communities in zhabei practiced ＂ strip-block linkage＂ on diabetes management. In the staffs, above 46 years old accounted for 48.2%, the specialized middle school accounted for 50.0%, primary title was 56.4%. 62.7% thought following-up sinked to the stand, the doctor-patients familiar degree ＂ strengthened obviously＂ or ＂strengthened＂. 59.8% of the providers thought they could mostly or completely to offer to the patients health instructions of ＂the clinical care, blood sugar monitor, medication＂ , and 57.6% ＂the personalized diet and exercises plan＂ .77.2% could ＂analyze and feeds back the patient condition control situation promptly＂ . Above three skills of the stand clinical doctors and nurses were all significantly better than that of public health care doctors. 35.8% thought the quality control had made improvement proposals to the follow-up, 26.9% and 4.6% thought that quality control target objectivity was ＂good＂ or ＂very good＂ . 3.6% and 27.3% thought that the patient compliance was ＂very good＂ or ＂good＂ , 21.8% thought patients welcomed the follow-up. CONCLUSION The main patters of diabetes community management in zhabei showed the superiorities of higher working efficiency, better doctor-patient relations, further general practice service, strengthened quality control. At the same time, there were also some problems in the work.
