溶血磷脂酸在冠心病患者中的表达及其与冠状动脉病变的关系

Association between plasma lysophosphatidic acid expression and coronary atherosclerotic severity in patients with coronary heart disease

摘要 目的观察溶血磷脂酸(LPA)在冠心病患者中的表达及其与冠状动脉病变的关系,并探讨LPA与炎症和脂质过氧化反应的关系。方法将按照美国心脏病学会/美国心脏协会冠心病处理指南的诊断标准并经冠状动脉造影确诊的85例患者纳入研究,分为3组,急性心肌梗死(AMI)组31例,不稳定型心绞痛(UA)组30例,稳定型心绞痛(SA)组24例;选择冠状动脉造影正常的20例非冠心病患者作为对照组。入选者均测定其LPA、C-反应蛋白(CRP)及低密度脂蛋白胆固醇(LDL-C)水平,并进行冠状动脉造影,按照Jenkins评分系统进行评分。比较各组LPA的表达情况,并对LPA与Jenkins评分、CRP、LDL-C进行Spearman直线相关分析。结果 4组患者LPA的表达水平比较,差异有统计学意义(P<0.01)。AMI组LPA的表达水平[(4.85±0.36)μmol/L明显高于UA组(3.67±0.84)μmol/L]、SA组[(2.61±0.73)μmol/L]和对照组[(1.75±0.49)μmol/L],UA组LPA表达水平明显高于SA组和对照组,SA组表达水平明显高于对照组。冠心病患者LPA的表达水平与Jenkins评分(r=0.901)、CRP水平(r=0.755)和LDL-C水平(r=0.549)均呈正相关,差异有统计学意义(P<0.01)。结论随着冠心病病情加重,LPA的表达水平升高,LPA的表达水平可反映冠状动脉病变的严重程度,LPA与炎症和脂质过氧化反应有关。 Objective To investigate the association between Plasma Lysophosphatidic expression and coronary atherosclerotic severity inpatients with coronary heart disease as well as the roles of LPA in inflammation and lipidperoxidation.Methods Eighty-five patients were diagnosed as CHD according to the results of coronary angiography and ACC/AHA diagnostic criteria.The patients were divided into 3 groups:acute myocardial infarction(AMI) group(n=31),unstable angina(UA) group(n=30),stable angina(SA) group(n=24).Another 20 patients with normal coronary artery served as control.All patients underwent coronary angiography and the results were further evaluated by Jenkins score.Expression of LPA and serum CRP were measured as well as LDL-C.Results The expression of LPA were significantly higher in CHD patients compared to controls(P〈0.01)in order of AMI group[(4.85±0.36)μmol/L] 〉UA group[(3.67±0.84)μmol/L vs AMI,P〈0.01]〉 SA group[(2.61±0.73)μmol/L vs AMI and UA,P〈0.01] 〉SA group[(1.75±0.49)μmol/L vs AMI,UA and SA,P〈0.01].The expression of LPA were positively correlated with Jenkins score(r=0.901,P〈0.01),CRP(r=0.755,P〈0.01) and LDL-C(r=0.549,P〈0.01).Conclusion Expression of LPA was significantly increases in patient with CHD and positively with coronary heart disease severity and serum CRP and LDL-C.
