摘要
目的:探讨早期肠内免疫营养对烫伤大鼠炎症反应的影响及对肠黏膜屏障功能的保护作用。方法:SD雄性大鼠54只,制成烫伤动物模型(30%TBSAⅢ度烫伤),随机分为标准营养组(EN组)和免疫营养组(EIN组),每组24只,EIN和EN组给予等热量肠内营养液,各组于伤后2h开始灌喂营养液。另取6只正常大鼠作为伤前对照(Control)。分别于伤前、伤后1、4、7、10d,处死每组各6只大鼠并立即心脏取血,检测血清中TNF-α、IL-2、IL-6、IL-8及D-乳酸的含量。结果:(1)烫伤后观察期内EIN组和EN组大鼠血清TNF-α、IL-6、IL-8水平均明显高于Control组(P<0.01或P<0.05),而IL-2水平则低于Control组(P<0.01或P<0.05),但EIN组于伤后7d恢复(P>0.05);伤后4、7、10dEIN组血清TNF-α、IL-6和IL-8水平显著低于EN组(P<0.05),而IL-2则明显高于EN组(P<0.05);(2)烫伤后血清D-乳酸水平升高,EN组在观察时间内血清D-乳酸水平没有恢复至伤前水平,EIN组在伤后4d恢复。结论:烫伤后早期给予肠内免疫营养,可减轻烫伤后大鼠全身炎症反应,同时改善肠黏膜的屏障功能,效果优于标准肠内营养。
Objective:To investigate the effects of enteral immunonutrition on inflammatory reaction and the protective effects for barrier function of intestinal mucosa in rats after thermal injury.Methods:Fifty-four SD rats inflicted by 30% TBSA Ⅲ degree scalds were randomly divided into enteral nutrition (EIN,n=24) group and enteral immunonutrition (EN,n=24) group.Another 6 normal rats were chosen as control group.The rats in EN and EIN groups were fed with isocaloric enteral nutrition in 2 hours after injury.Each 6 rats of three groups were sacrificed at 1^st,4^th,7^th and 10^th day before and after injury.The heart blood samples were collected immediately to detect the concentrations of TNF-α,IL-2,IL-6,IL-8 and D-lactic acid in serum.Results:(1)The serum level of TNF-α,IL-6 and IL-8 in both EIN and EN groups were obviously higher than those in control group (P〈0.01 or P〈0.05) but the level of IL-2 was decreased (P〈0.01 or P〈0.05).The rats in EIN recovered at 7^th day (P〉0.05) after injury and the expression of TNF-α,IL-6 and IL-8 were significantly lower than those in EN (P〈0.05) at 4^th,7^th and 10t^h day.However,the level of IL-2 was obviously increased than the one in EN (P〈0.05) at same intervals.(2)After injury,the serum level of D-lactic acid increased in control group,which was not reach the same leve l before injury in observation period in EN group and get to the normal level after 4 day in EIN group.Conclusion:Enteral immunonutrition superior to enteral nutrition in the early stage of scalded injury could relieve inflammatory reaction of rats and simultaneously improved the barrier function of intestinal mucosa.
