摘要
目的:观察预防性应用卡维地洛对肺动脉高压致右心衰竭幼鼠血流动力学、右心室功能及血浆B型脑钠肽(BNP)的影响。方法:取44只SD幼鼠。Ⅰ:取18只幼鼠。模型1组(M1组)10只,腹腔注射10g/L野百合碱溶液诱发肺动脉高压致右心衰竭模型;正常对照1组(N1组)8只,同法给予等体积生理盐水。4周末测定幼鼠血流动力学指标包括中心静脉压(pCV)、右心室收缩压(pRVS)、右心室压最大上升速率(+RVdp/dt)、平均主动脉压(pMA)和左心室压最大上升速率(+LVdp/dt)及右心功能包括右室肥厚指数[(RV/(LV+S)]和左室质量与体质量的比值(LV/BW)。Ⅱ:余26只随机分为3组,模型2组(M2组,n=8)和卡维地洛组(C组,n=10)同上建立右心衰竭模型,C组于腹腔注射野百合碱溶液后2周给予卡维地洛4周,正常对照2组(N2组,n=8)仅给予等体积生理盐水4周。6周末同上测定幼鼠血流动力学指标与右心功能,采用ELISA法测定幼鼠血浆BNP水平。结果:Ⅰ:与N1组相比,M1组pCV与pRVS升高,+RVdp/dt降低,RV/(LV+S)增加(t=9.213、5.127、7.720和10.265,P均<0.05)。Ⅱ:C组、M2组和N2组pCV、pRVS、+RVdp/dt、RV/(LV+S)及血浆BNP水平相比,差异均有统计学意义(F=19.086、8.359、8.166、11.810和10.057,P均<0.05)。C组pCV与pRVS低于M2组,+RVdp/dt高于M2组,RV/(LV+S)低于M2组,血浆BNP水平低于M2组(P均<0.05)。结论:预防性应用卡维地洛能减缓肺动脉高压所致右心衰竭的形成和发展。
Aim:To research the effects of prophylactic application of carvedilol on the hemodynamics,right ventricular function,and plasma B-type natriuretic peptide(BNP) level in juvenile rats with right heart failure(RHF) caused by pulmonary hypertension(PH).Methods:A total of 44 SD juvenile rats were used. PartⅠ:18 of them were randomly chosen and allocated into model group 1(group M1) and normal control group 1(group N1). Ten rats in group M1 were injected with peritoneal cavity with monocrotaline to induce PH and RHF.Eight rats in group N1 were injected with normal saline.The hemodynamic parameters [central venous pressure(pCV),right ventricular systolic pressure(pRVS),right ventricular pressure maximum upstroke velocity (+RVdp/dt),mean aortic pressure(pMA) and left ventricular pressure maximum upstroke velocity(+LVdp/dt)],right ventricular pachynsis index[RV/(LV+S)] and weight ratio of left ventricular and body(LV/BW) were observed at the end of 4 weeks. PartⅡ:26 rats were randomly allocated into model group 2(group M2),carvedilol group (group C) and normal control group 2(group N2).Group M2 and C were established RHF model by the same method.The rats in group C were administered carvedilol for four weeks via intragastric route 2 weeks after monocrotaline administration.The rats in group N2 were only given normal saline. The hemodynamics and function of right heart were observed. The plasma BNP level was determined by ELISA method.Results:In experiment Ⅰ,pCV and pRVS in group M1 were higher than those in group N1,+RVdp/dt was lower,and RV(LV+S) increased(t=9.213,5.127,7.720 and 10.265,P0.05). In experiment Ⅱ,the differences of pCV,pRVS,+RVdp/dt,RV/(LV+S) and the plasma BNP level among group C,M2 and N2 were significant (F=19.086,8.359,8.166,11.810 and 10.057,P0.05). pCV and pRVS in group C were significantly lower than those in group M2,+RVdp/dt was higher and RV/(LV+S) was lower than those in group M2(P0.05). Also the plasma BNP level in group C was lower than that in group M2(P0.05).Conclusion:The prophylactic application of carvedilol could delay the occurrence and development of RHF with PH.
