摘要
目的研究姜黄素眼膏对兔实验性小梁切除术后滤过道瘢痕的影响。方法将27只新西兰大白兔按体质量分层随机分为对照组、基质组、姜黄素组,每组9只,均选右眼行标准小梁切除术。术后对照组不做特殊处理;基质组给予约0.1g眼膏基质,3次/d;姜黄素组给予约0.1g姜黄素眼膏,3次/d。分别观察3组眼压、滤过泡、炎性细胞、胶原、成纤维细胞和肌成纤维细胞。结果术前,术后1、3、28d3组眼压值差异不明显(P>0.05),术后7、14、21d3组眼压值有明显差异(P<0.05)。术后7d时姜黄素组与对照组和基质组间的炎性细胞比较差异有统计学意义(P<0.05)。而术后14、28d3组间比较差异无统计学意义(P>0.05)。姜黄素组各个时间点的I型胶原量[7d(21.62±2.87)%,14d(27.83±2.37)%,28d(33.47±2.79)%]与对照组[7d(29.71±1.31)%,14d(36.83±1.73)%,28d(47.13±1.85)%]和基质组[7d(31.07±1.67)%,14d(38.09±1.17)%,28d(48.13±1.69)%]比较差异有统计学意义(P<0.05),但Ⅲ型胶原差异无统计学意义(P>0.05)。姜黄素组的成纤维细胞和肌成纤维细胞与对照组和基质组比较有显著差异(P<0.05)。结论姜黄素眼膏可抑制成纤维细胞和肌成纤维细胞增生,减少Ⅰ型胶原分泌,有效延缓滤过泡瘢痕化进程。
Objective To study the effect of curcumin ointment on scar formation in filtering blebs after trabeculectomy in rabbits. Methods Twenty-seven rabbits( 27 eyes) were randomly divided into control group,and matrix( placebo) group,and curcumin group,9 in each group. Trabeculectomy for right eyes was performed on all rabbits. Eyes in control group were not treated with any drug,eyes in matrix group were smeared with 0. 1 g common eye ointment ( 3 times a day) ,and eyes in curcumin group were smeared with 0. 1 g curcumin eye ointment ( 3 times a day). Then,intraocular pressure ( IOP) ,filtering blebs,inflammatory cells,collagen,fibroblasts and myofibroblasts were detected by immunohistochemical staining. Results No significant difference was observed in IOP among the 3 groups before and 1,3,28 d after operation. However,a significant difference was found in IOP among the 3 groups in 7,14 and 21 d after operation ( P 0. 05). The number of inflammatory cells was larger in curcumin group than in matrix and control groups in 7 d after operation ( P 0. 05). However,the number of inflammatory cells was similar in 3 groups in 14 and 28 d after operation. In 7,14 and 28 d after operation,the type 1 collagen accounted for ( 21. 62 ± 2. 87) % ,( 27. 83 ± 2. 37) % ,and ( 33. 47 ± 2. 79) % in curcumin group,( 29. 71 ± 1. 31) % ,( 36. 83 ± 1. 73) % ,and ( 47. 13 ± 1. 85) % in control group,and ( 31. 07 ± 1. 67) % ,( 38. 09 ± 1. 17) % ,and ( 48. 13 ± 1. 69) % in matrix group,respectively,( P 0. 05). No difference was found in type 3 collagen. The number of fibroblasts and myofibroblasts was larger in curcumin group than in matrix and control groups ( P 0. 05). Conclusion Curcumin eye ointment can inhibit the proliferation of fibroblasts and myofibroblasts,reduce the secretion of type 1 collagen,and delay the scar formation in filtering blebs.
