摘要
目的探讨不同用法的缩宫素在剖宫产术中的应用效果。方法选择80例足月子宫下段剖宫产孕妇,随机分为两组,其中一组40例为观察组,于胎儿娩出后子宫肌注缩宫素10U,同时由另一通道静脉滴注缩宫素10U;另一组40例为对照组,于胎儿娩出后子宫肌注缩宫素10U,同时静脉推注缩宫素10U。观察两组术中子宫收缩、出血量、血压、心率的变化和药物不良反应。结果两组术中子宫收缩、出血量差异无统计学意义,用药后5min血压均下降,心率升高,其中血压下降幅度对照组显著高于观察组(P<0.01),但对照组心率升高幅度与观察组相比差异无统计学意义;用药后10min,血压、心率均升高,其中血压升高幅度对照组显著高于观察组(P<0.01),心率升高幅度对照组高于观察组(P<0.05),不良反应发生率对照组明显高于观察组(P<0.01)。结论剖宫产术中相同剂量的缩宫素静脉滴注,在单位时间内作用于体内的药物浓度相对较低,既能保持缩宫素收缩子宫的作用,又能有效的预防和减轻其心血管不良反应,使剖宫产手术更为安全。
Objective To investigate the usage of oxytocin during cesarean section in the application results.Methods 80 cases of lower uterine segment cesarean section in term pregnant women were randomly divided into two groups,one group(40 cases)for the observation group,in the uterus after the baby is delivered intramuscular injection of oxytocin 10 U,while the other channel by intravenous drip oxytocin 10 U,the second group(40 cases)as the control group,in the uterus after the baby is delivered intramuscular injection of oxytocin 10 U,while intravenous injection of oxytocin 10 U.Surgery were observed in the contraction of the uterus,bleeding volume,blood pressure,heart rate changes and adverse drug reactions.Results 2 patients in the contraction of the uterus,no significant difference between the amount of bleeding,5 min after administration decreased blood pressure,heart rate increased,which decrease blood pressure in control group was significantly higher than the observation group(P〈0.01),but the control group,heart rate higher rate of no statistical significance;medication after 10 min,blood pressure and heart rate were increased,in which blood pressure rises in the control group was significantly higher than the observation group(P〈0.01),heart rate rises in the control group was higher than the observation group(P〈0.05),incidence of adverse reactions were significantly higher than the observation group(P〈0.01).Conclusion The cesarean section in the same dose of intravenous infusion of oxytocin in the unit time acting on the body drug concentrations are relatively low,both oxytocin contraction to maintain the role of the uterus,but also effective prevention and mitigation of their Cardiovascular adverse reactions,so that cesarean section is more secure.
