摘要
目的探讨浅表性膀胱癌术后采用吡柔比星（THP）膀胱灌注预防肿瘤复发的临床效果。方法对38例浅表性膀胱癌患者自术后第1周起用吡柔比星30 mg膀胱灌注,隔日1次,共6次;之后改为2周1次,共4次;4周1次,共4次;8周1次,持续至1 a。随访6个月~2 a,观察肿瘤复发率和不良反应情况。结果术后1 a内无肿瘤复发,1 a后肿瘤复发3例（8%）。21例（55%）出现尿频、尿急、尿痛、血尿和膀胱区疼痛;无须治疗,5 d后症状消失;6例（11%）出现严重的膀胱痉挛,给予托特罗定和/或灌注时加利多卡因症状消失;2例（5%）膀胱挛缩而中断灌注,给予托特罗定、膀胱训练治愈;2例（5%）出现尿道狭窄,给予定期尿道扩张排尿通畅。结论THP膀胱灌注预防膀胱肿瘤复发疗效显著,但要积极预防和处理并发症。
Objective It is to approach the prevention effect of bladder tumor recurrence by Pirarubicin bladder irrigation in superficial bladder cancer postoperative.Methods Since the first week postoperative,38 patients with superficial bladder cancer were treated with Pirarubicin 30 mg by bladder instillation,until 1 year.Followed up 6 months to 2 years,the tumor recurrence rate and side effect were observed.Results There was on recurrence within one year,3 cases（8%） recurred after one year.21 cases（55%） showed up symptoms such as frequent micturition,urgency,urination pain,hematuria and bladder pain,and the symptoms disappeared in 5 days without treatment.6 cases（11%） with severe bladder spasticity treated with tolterodine and/or add lidocaine in perfusion.2 cases（5%） of bladder contracture and perfusion interrupted treated with tolterodine and bladder training for curable.2 cases（5%） of urethral stricture were given regular urethral smooth expansion.Conclusion The effect of Pirarubicin bladder irrigation to prevent bladder tumor recurrence was significant,but to actively prevent and deal with complications.
