摘要
目的:探讨凋亡相关基因survivin在非小细胞肺癌中表达及其与bcl-2的关系。方法:选取52例非小细胞肺癌患者组织,其中鳞癌26例、腺癌24例、大细胞未分化癌2例。正常肺组织10例,分别应用免疫组织化学法检测survivin及bcl-2蛋白的表达。结果:survivin表达率在正常肺组织为0,非小细胞肺癌为65.39%,2组比较差异有统计学意义;在高分化癌组织为44.44%,在中-低分化癌组织为76.47%,2组比较差异有统计学意义;在有淋巴结转移组为82.61%,无淋巴结转移组为51.72%,2组比较差异有统计学意义。Survivin的表达与患者年龄、性别、吸烟史及病理分型无关。survivin在bcl-2阳性表达组和阴性表达组中的表达率分别为90.00%和37.93%,差异有统计学意义。结论:survivin基因异常表达引起的细胞凋亡抑制在非小细胞肺癌发生中起重要作用,其过度表达提示非小细胞肺癌预后不良;survivin蛋白表达与bc1-2蛋白表达呈正相关,二者在非小细胞肺癌发生中可能起协同作用。联合检测survivin蛋白和bc1-2蛋白,可为判断非小细胞肺癌侵袭、转移的能力、预后提供依据。
Objective To investigate the expression and clinical significance of survivin and bcl-2 in non-small cell lung carcinoma.Methods The expressions of survivin and bcl-2 were detected with immunohistochemical analysis in 52 cases of non-small cell lung carcinoma(26 cases of squamous celled carcinoma,24 cases of adencarcinoma and 2 cases of large cell undifferentiated carcinoma) and 10 cases of normal lung specimens.Results The positive rates of survivin expressed in normal lung and non-small cell lung carcinoma tissues were 0 and 65.39% respectively,which showed a significant difference between the two groups.The positive expression rate of survivin in highly differentiated cancer tissues(44.44%) was prominently lower than that in moderately and poorly differentiated cancer tissues(76.47%).The expression of survivin in patients with lymph node metastasis(82.61%) was significantly higher than that in patients without lymph node metastasis(51.72%).The expression of survivin was not associated with clinicopathological factors,including age,sex,smoking,pathological type,etc.The positive rate of survivin expression was obviously higher in positive bcl-2 than in negative bcl-2 cases(90.00% vs 37.93%),which showed a significant difference between the two groups(P0.01).Conclusion Apoptosis inhabitations caused by abnormal survivin expression may participate in the development and progression of non-small cell lung carcinoma.The over-expression survivin indicates the poor prognosis.The survivin protein expression is positively correlated to and has synergism with bcl-2 protein expression in non-small cell lung cancer.Joint detection of survivin protein and bcl-2 protein can make a judgement of the capability of invasion and metastasis,and the prognosis of non-small cell lung cancer.
