高原缺氧对急进高原个体胃肠动力激素及其粘膜屏障的影响 被引量:28

摘要 目的:对高原缺氧对急进高原个体胃肠动力激素及其粘膜屏障的影响进行探讨。方法:对20例由平原急进高原个体于进入高原前、进入高原第3天和第7天胃动素(MTL)、血管活性肠肽(VIP)、P物质(SP)、胰高血糖素样肽(GLP-1)及胃泌素(GAS)的含量进行检测,同步进行胃镜检查,并对进入高原后消化系统的症状发生情况进行调查。结果:本组20例急进高原个体胃肠动力激素MTL、VIP、SP、GLP-1及GAS均出现显著改变,其中MTL、SP显著降低,VIP、GLP-1及GAS显著升高,进入高原后第3天及进入高原后第7天与进入高原前比较,分别为P<0.01和P<0.05。胃镜下明显存在胃蠕动缓慢及胃粘膜改变,进入高原第3天及第7天与进入高原前比较均具有显著差异(分别为P<0.01和P<0.05),其中进入高原第3天胃蠕动缓慢及粘膜改变分别高达60%、85%。并且20例急进高原个体中有13例存在不同程度的消化系统症状(占65%),症状主要有腹胀、食欲不振、恶心、呕吐、腹痛、便秘、黑便等,其中腹胀、食欲不振、恶心、呕吐分别高达60%、40%、25%和10%。结论:高原急性缺氧对急进高原个体胃肠动力激素存在明显的影响,并因此而出现胃肠运动缓慢,从而导致急性胃肠粘膜损伤。 Objective:Analysis the effect of acute high altitude hypoxia exposed to motive hormones and mucosal barrier of gastric and intestinal.Methods: take examination of gastroscopy for 20 people who entered into plateau rapidly in I day before they entered into plateau and 3,7 days after they entered into plateau separately,then draw blood and detected the content of motilin,vasoactive intestinal peotide,glucagon-like peptide,gastrin and substance P in peripheral blood serum at the same time,then investigate the happen of digesting symptoms after entered into plateau.Result: there were significant decrease of MTL and SP after hypoxia exposed inplateau,and VIP,GLP-I and GAS17 were decreased at the same time,the changes are more dramatically in 3 days(P〈0.01) than 7 days(P〈0.05).There existed slow wriggling of stomach(60%)and alteration of gastric mucosal(85%) showed by gastroscopy.Digesting symptoms included abdominal distension,poor appetite,nausea vomiting,abdominal pain,constipation and dark stools,the proportion of abdominal distension,poor appetite,nausea and vomiting are 60%,40%,25% and 10% separately.Conclusion:acute expose of hypoxia in plateau can cause significant change of gastric and intestinal motive hormones,and lead to slow movement of stomach and intestinal,then result in acute damage of gastric and intestinal mucosal.
