摘要
目的检测白细胞介素-4(IL-4)受体α链(IL-4Rα)I50V和Q576R基因多态性与呼吸道合胞病毒(RSV)毛细支气管炎的相关性。方法采用聚合酶链-限制性片段长度多态法检测130例RSV毛细支气管炎患儿和108例健康儿童IL-4Rα/I50V、Q576R基因多态性,应用化学发光法和酶联免疫吸附法检测血清总IgE和鼻咽分泌物(NPS)IL-4R水平。结果病例组II、IV和VV基因型频率为24.6%、51.5%和23.9%,对照组为27.8%、40.7%和31.5%,差异无统计学意义(χ2=2.960,P>0.05);病例组I、V等位基因频率为50.4%、49.6%,对照组为48.1%、51.9%,差异无统计学意义(χ2=0.236,P>0.05),II基因型NPS中IL-4R高于IV和VV基因型(Z=2.031,2.034,P<0.05)。病例组QQ和QR基因型频率为72.3%、27.7%,对照组为64.8%、35.2%,差异无统计学意义(χ2=0.521,P>0.05);病例组Q、R等位基因频率为86.2%、13.8%,对照组为82.4%、17.6%,差异无统计学意义(χ2=1.261,P>0.05),R等位基因携带者NPSIL-4R高于Q等位基因携带者(Z=2.205,P<0.05)。两个位点基因型间血清总IgE差异无统计学意义(F=1.021,t=0.452,P>0.05);基因型频率与病情间亦无关联(χ2=0.322,1.393,P>0.05)。结论 IL-4Rα/50II基因型及576R等位基因携带者与RSV毛细支气管炎NPSIL-4R水平增高相关。
Objective:To investigate the association between interleukin 4 receptor alpha (IL-4Rα) gene polymorphisms and the susceptibility to and the severity of respiratory syncytial virus (RSV) bronchiolitis.Furthermore,to explore the impact of IL-4Rα gene polymorphisms upon IL-4R in nasopharyngeal secretions (NPS) and total serum IgE level.Methods:Genotypes of I50V and Q576R were determined by polymerase chain reaction-restriction fragment length polymorphism analysis(PCR-RFLP) in 130 children with RSV bronchiolitis and 108 controls.Total serum IgE and NPS IL-4R level were assayed with chemiluminescence assay and ELISA,respectively.Results:IL-4Rα/I50V gene polymorphism was present both in case and control group.The genotype frequency was 24.6% (II),51.5% (IV),23.9% (VV) in case group,and 27.8% (II),40.7% (IV),31.5% (VV) in control group.It showed no significant difference between two groups (χ^2=2.960,P〉0.05).The allele frequency was 50.4% (I),49.6% (V) in case group,and 48.1% (I),51.9% (V) in the controls.There was not significant difference between two groups (χ^2=0.236,P〉0.05).However,among case group,II genotype was significantly associated with high NPS IL-4R level compared with IV and VV genotypes (Z=2.031,2.034,P〈0.05).IL-4Rα/Q576R gene polymorphism was present both in case group and control group.The genotype frequency in case group was 70.0% (QQ),30.0% (QR),and 64.8% (QQ),35.2% (QR) in control group.It showed no significant difference between two groups (χ^2=0.521,P〉0.05).The allele frequency was 86.2% (Q),13.8% (R) in case group,and 82.4% (Q),17.6% (R) in the controls.There was not significant difference between two groups (χ^2=1.261,P〉0.05).R allele was associated with significantly high NPS IL-4R level in case group (Z=2.205,P0.05).No difference was found in total serum IgE level among three genotypes of I50V (F=1.021,P〉0.05) and between two genotypes of Q576R (t=0.452,P〉0.05).Furthermore,the genotype frequency of I50V and Q576R did not associated with the severity (χ^2=0.322,1.393,P〉0.05).Conclusions:II genotype of IL-4Rα/I50V and R allele of IL-4Rα/Q576R are both associated with high NPS IL-4R level in children with RSV bronchiolitis..
