血清尿酸对高血压前期人群血压转归的影响 被引量:23

The Assessment of Carotid Artery Remodeling by Quantitative Tissue Velocity Imaging in Essential Hypertension Patients with Different Left Ventricular Patterns

摘要 目的探讨血清尿酸对高血压前期人群血压转归的影响。方法选择2006-2007年开滦集团公司职工健康体检中符合美国高血压预防、检测、评估和治疗联合委员会第7次报告高血压前期诊断标准的高血压前期人群作为观察队列。于2008-2009年对观察队列进行第2次健康体检,观察血清尿酸与血压转归的关系。结果①共入选33913例高血压前期人群作为观察队列,排除随访期间内发生心脑血管事件者、死亡者、失访者及参数缺失者后,纳入最终统计的有效数据为25474例,在2年内共有8358例进展至高血压。按血清尿酸四分位分组,结果显示随尿酸水平升高,进展至高血压的比例增加(26.2%、30.9%、34.7%、39.3%),差异有统计学意义。②多元线性回归分析显示,血清尿酸每增加1个标准差,收缩压升高0.444mmHg,舒张压升高0.353mmHg。③多因素logistic回归分析显示血清尿酸四分位第2、3、4组进展至高血压的风险(RR)分别是第1组的1.26、1.49、1.83倍。校正体质量指数、吸烟史、饮酒史、基线收缩压、基线舒张压后,血清尿酸第4四分位组进展至高血压的风险有所降低(RR=1.24,95%CI1.14~1.35),但仍有统计学意义。结论血清尿酸增加是高血压前期人群进展至高血压的独立危险因素。 Objective To evaluate the relationship between serum uric acid (SUA) and the progression of blood pressure in patients with prehypertension. Methods A cohort of 33 913 patients with prehypentension in Kailuan hospital was recruited in 2006-2007 and followed up for 2 years to examine the possible effect of SUA on blood pressure. Results ①There were 25 474 patients involved in the final statistics excluding patients who died or were lost during follow-up. A total of 8358 patients with prehypentension developed hypertension in 2 years. The hypertension incidence rate was increased with the SUA levels,which were 26.2%, 30. 9%, 34.7%, 39.3% in patients with SUA level of quartile 1, quartil 2, quartil 3, quartil 4 respectively. ② Multiple linear regression showed systolic blood pressure (SBP) and diastolic blood pressure (DBP) increased 0. 444 mm Hg and 0. 353 mm Hg respectively with the increasing of 1 standard deviation of SUA. ③Multiple logistic regression models showed that patients in quartile 2, quartile 3 or quartile 4 group carried 1.26, 1.49 or 1.83 times greater risk for hyperten- sion respectively than patients in the lowest SUA quartile group (quartile 1). After adjustment for body mass in- dex, smoking, drinking, baseline SBP and baseline DBP, the risk for hypertension decreased with increasing level of SUA, but still remained statistically significant higher(RR = 1.24, 95 % CI 1.14- 1.35). Conclusion Elevated SUA is an independent risk factor for hypertension in patients with prehypertension.
