摘要
目的探讨应用改良Millard法修复单侧唇裂并同期矫正鼻畸形的临床效果。方法在传统Millard术式旋转推进法修复唇裂的基础上进行改良，将鼻小柱的皮肤定点由传统的健侧位移至近患侧鼻小柱根部，移动幅度根据患侧鼻孔大小及鼻小柱畸形程度适当调整；不再设计患侧鼻底及鼻翼基部水平切口，改为充分分离错位的患侧口轮匝肌，使其与健侧复位后的口轮匝肌对位缝合；同时通过鼻小柱、鼻翼基部切口分离患侧鼻翼软骨，将其与对侧上鼻翼软骨及皮肤悬吊固定，矫正鼻翼塌陷。自2005年10月至2009年5月，采用本法矫正单侧唇裂及鼻畸形患者46例。结果本组46例患者切口均I期愈合。经1～24个月随访，上唇、人中嵴解剖形态自然，动态和静态效果良好，鼻畸形得到明显改善。结论应用改良Millard方法修复单侧唇裂（完全性及不完全性）并同时矫正鼻畸形，是一种综合修复单侧唇裂的良好方法。
Objective To explore the clinical effect of primary repair of modified Millard＇s repairing on unilateral cleft lip combined with nasal deformity reconstruction. Methods The basic principle of primary unilateral cleft lip repair was built on tissue rotation and advancement concept by Millard. Individualized colmnella landmark was designed for the reconstruction of nostril. Museulus orbicularis oris on the cleft side was separated adequately and apposition suture was performed in stead of the traditional horizontal incision on nasal basis and nasal ala. Suspension was operated on nasal alar cartilage and skin after separation for correction of nasal alar collapse by incision on columella nasi and nasal ala basis. From October 2005 to May 2009, 46 patients with u- nilateral cleft lip was treated by this technique. Results Fourty-six patients all healed primarily. After 1 24 months follow-up, satisfactory results were received with optimal dynamic and static appearance of upper lip and phihrum column, also nasal deformity was improved markedly. Conclusion It is a synthetic and effective method for the treatment of unilateral cleft lip and nasal deformity by modified Millard＇s repairing.
