Objective To study the relationship between ischemic areas in the microcirculation and injury of the vascular endothelial cells in patients with stage Ⅱ essential hypertension. Methods Sixty patients with essential hypertension at stage Ⅱ and twenty normotensive persons were observed for the bulbar conjunctiva microcirculation. The circulating endothelial cells (CEC),plasma endothelin (ET),and plasma endothelium derived relaxing factor (EDRF) were measured. Results The incidence of ischemic area in patients with essential hypertension was 53.3% higher than that in normotensive persons (P<0.001).The CEC and plasma ET significantly increased (P<0.001) and the plasma EDRF markedly reduced in patients as compared with the normotensive group. It has been found that in patients with essential hyperension, ischemic areas in the microcirculation were closely related to dysfunction of the vascular endothelial cells. Conclusion They acted each other and became more and more serious, forming a vicious cycle that may play an important role in the pathogenesis and development of essential hypertension.
