摘要
目的研究铜绿假单胞菌临床分离株生物膜形成情况,及L-精氨酸协同妥布霉素和环丙沙星对包被于成熟生物膜内铜绿假单胞菌的抗菌活性。方法选取2008年1月-2009年8月临床分离的铜绿假单胞菌96株,用半定量微量板法检测铜绿假单胞菌生物膜形成情况,用琼脂稀释法检测生物膜形成株对环丙沙星和妥布霉素的MIC值,采用半定量微量板法检测环丙沙星或妥布霉素以及L-精氨酸协同环丙沙星或妥布霉素对生物膜形成株的影响。结果 96株铜绿假单胞菌中有42株在体外形成生物膜,阳性率为42.9%,42株生物膜形成株均为多药耐药;生物膜形成阳性株对头孢他啶、头孢哌酮/舒巴坦、亚胺培南和哌拉西林/他唑巴坦的耐药明显高于生物膜形成阴性株;环丙沙星组和环丙沙星加L-精氨酸组的生物膜形成的OD值分别为0.431±0.187和0.227±0.060,加入L-精氨酸后能明显提高环丙沙星对生物膜细菌的抑菌作用(P<0.01);妥布霉素组和妥布霉素加L-精氨酸组的生物膜形成的OD值分别为0.323±0.111和0.249±0.087,加入L-精氨酸后能明显提高妥布霉素对生物膜细菌的抑菌作用(P<0.01)。结论铜绿假单胞菌临床分离株生物膜形成阳性率较高,形成生物膜后导致耐药率升高,L-精氨酸能够提高环丙沙星和妥布霉素对生物膜形成株的抑菌作用。
OBJECTIVE To investigate the prevalence of biofilm formation in clinical isolates of Pseudomonas aeruginosa and the influence of L-arginine with ciprofloxacin and tobramycin on the biofilm formation of P.aeruginosa clinical isolates.METHODS Ninety-six isolates of P.aeruginosa were isolated from clinical specimens from Jan 2008 to Aug 2009.Microtiter-plate test was performed to semi-quantify biofilm formation among P.aeruginosa isolates.Broth microdilution test was used for determining the MICs of ciprofloxacin and tobramycin of biofilm-forming isolates.Microtiter-plate test was also used for determining the antimicrobial activity of ciprofloxacin with or without L-arginine and tobramycin with or without L-arginine to biofilm-formating isolates.RESULTS Forty-two(42.9%) of 96 clinical isolates of P.aeruginosa were biofilm-forming isolates.All biofilm-forming isolates were multiple resistant to antibiotics.The resistant rates of biofilm-forming isolates to ceftazidime,cefoperazone/sulbactam,imipenem and piperacillin/tazobactam were higher than those to non biofilm-forming isolates.The OD values of ciprofloxacin group,ciprofloxacin plus L-arginine group,tobramycin group and tobramycin plus L-arginine group were 0.431±0.187,0.227±0.060,0.323±0.111 and 0.249±0.087.Compared with the values of ciprofloxacin group and tobramycin group,the values of ciprofloxacin plus L-arginine group and tobramycin plus L-arginine group were significantly decreased(P〈0.01).CONCLUSION The prevalence of biofilm formation of P.aeruginosa clinical isolates is high.The resistance of biofilm-forming isolates is serious.L-Arginine can improve the antimicrobial activity of ciprofloxacin and tobramycin to biofilm-forming P.aeruginosa clinical isolates.
