多药耐药铜绿假单胞菌消毒剂、灭菌剂耐药基因研究 被引量:5

Resistance Genes to Disinfectants and Sterilants in Multi-drug Resistant Strains of Pseudomonas aeruginosa

摘要 目的调查6种消毒剂、灭菌剂耐药基因(smr、smr-2、qacE△1t、ehA、oqxA、merA)在铜绿假单胞菌分离株中的存在和变化情况。方法收集复旦大学附属上海市第五人民医院2009年5-11月分离自住院患者送检痰液标本的铜绿假单胞菌共20株,采用聚合酶链反应(PCR)及序列分析的方法分析6种消毒剂、灭菌剂耐药基因。结果 20株铜绿假单胞菌中,smr基因均有检出(100%),qacE△1基因和merA基因阳性株数分别为12株(60%)和11株(55%),而smr-2t、ehA、oqxA基因未检出;3种阳性基因以3种阳性模式存在,其中3种基因同时检出的模式:smr+qacE△1+merA检出率最高,共11株(55%);3号株merA基因与美国NCBI中已登录的序列均不同,为新亚型。结论该组铜绿假单胞菌检出消毒剂、灭菌剂耐药基因,且阳性率高,提示该组菌多数已耐消毒剂、灭菌剂,所以要严格控制对泛用消毒剂、灭菌剂的使用;铜绿假单胞菌进行6种消毒剂、灭菌剂耐药基因检测研究为国内首次,检出smr基因亦为国内首次报道。 OBJECTIVE To investigate the distribution and variety of 6 kinds of resistance genes to disinfectants and sterilants(smr,smr-2,qacE△1,tehA,oqxA,and merA)in multi-drug resistant strains of Pseudomonas aeruginosa.METHODS From May 2009 to Nov 2009,20 strains of multi-drug resistant P.aeruginosa isolated from sputum were collected from Shanghai No.5 Hospital affiliated to Fudan University,China.Six kinds of resistance genes to disinfectants and sterilants were analyzed by PCR and verified by DNA sequencing.RESULTS In 20 strains of P.aeruginosa,20 strains(100%),12 strains(60%) and 11 strains(55%)were detected to carry smr,qacE△1 and merA,respectively.However,smr-2,tehA and oqxA couldn′t be detected.Furthermore,3 kinds of positive models were classified according to the presence of 3 kinds of positive genes.Among them,the model smr+qacE△1+merA had the highest detection rate(11 strains,55%).In addition,after genomic comparison with sequences registered in NCBI,the genotype of merA carried on strain No.3 was confirmed as a new subtype.CONCLUSIONS The fact that resistance genes to disinfectants and sterilants are popular in P.aeruginosa,suggested the most strains of this group of P.aeruginosa be resistant to disinfectants and sterilants.Therefore,widely used disinfectants and sterilants should be strongly controlled.It′s the first report to detect 6 kinds of resistance genes to disinfectants and sterilants simultaneously in P.aeruginosa in China.And it′s the first report that smr was detected in China.
