摘要
目的评价降钙素治疗对早期股骨头坏死病人防止股骨头塌陷的疗效。方法共90例患者入组,随机分为两组:治疗组给予鲑鱼降钙素注射剂50IU肌注,每日一次,30天为一疗程,患者共接受3个疗程的治疗;对照组不积极干预,仅使用止痛药物,减少负重。患者治疗前后进行疼痛评分和Harris评分,拍X光片,行MRI检查,并对股骨头坏死和塌陷进展进行观察。结果所有患者在治疗结束后平均随访36个月,治疗组42例得到随访,37例Harris评分从治疗前71分到治疗后92分(P<0.05),疼痛评分治疗前平均8.7分下降到3.4分(P<0.05)。对照组39例得到随访,病情进展36例,已手术15例,Harris评分从治疗前80分下降到61分(P<0.05),疼痛评分治疗前平均8.1分上升到9分(P<0.05)。鲑鱼降钙素治疗组未发现严重不良反应。结论鲑鱼降钙素可以减缓股骨头坏死的进展,防止股骨头塌陷,减轻疼痛改善症状,对早期股骨头坏死治疗有一定疗效。
Objective To assess the prevention efficacy of salmon calcitonin in the collapse of femoral head in petients with early stage of avascular necrosis. Methods Ninety patients with early stage of the avascular necrosis of femoral neck were randomly assigned into two groups: the patients in treatment group were given 50 IU/d salmon calcitonin intramuscularly for 30 days for one course, and continued for 3 courses; the patients in control group were given no active treatment except pain killers and less weight-bearing. Patients were assessed by X-ray, MRI for the progression of the necrosis and collapse of femoral heads before and after the treatment. Results All patients were followed up for an average of 36 months. Of the 42 patients followed up in the treatment group, Harris score rose from 71 before treatment to 92 after treatment (P 〈 0.05) and VAS reduced from 8.7 to 3.4 (P 〈 0. 05) in 37 patients. Of the 39 patients followed up in the control group, disease deteriorated in 36 patients and surgery was performed in 15 patients. Their Harris score went down from 80 before treatment to 61 after treatment. VAS rose from 8.1 to 9 (P 〈 0. 05). No severe adverse events were found in salmon calcitonin treatment group during the study. Conclusion Treatment with salmon calcitonin slowed down the deterioration of avascular necrosis of femoral neck, prevented femoral neck from collapse, relieved pain. It had certain efficacy on the treatment of early stage necrosis of the femoral neck.
