摘要
目的探讨多囊卵巢综合征(PCOS)促排卵治疗中出现的卵泡未破裂黄素化综合征(LUFS)的治疗措施。方法选择2006年1月~2007年12月在临沂市妇幼保健院生殖中心就诊的PCOS不孕症患者经促排卵治疗,对发生LUFS 58例患者35例给予卵泡穿刺(为穿刺组),23例妇科检查机械性挤压法(为挤压法组)治疗,比较治疗后的排卵率、妊娠率以及中、重度卵巢过度刺激综合征(OHSS)的发生率。结果穿刺组排卵率为100%(35/35),妊娠率为31.4%(11/35),OHSS的发生率为2.9%(1/35);挤压组排卵率为47.8%(11/23),妊娠率为8.7%(2/23),OHSS的发生率为13.0%(3/23);两组妊娠率及OHSS发生率比较,差异无统计学意义;穿刺组排卵率高于挤压组,差异有统计学意义(P<0.001)。结论PCOS促排卵治疗中发生的LUFS给予卵泡穿刺术治疗效果肯定,是一种有效的治疗方法。
Objective To evaluate the treatment of luteinized unruptured follicle syndrome (LUFS) in patients with polyeystic ovary syndrome (PCOS) in ovulation induction. Methods Fifty-eight LUFS patients with PCOS in ovulation induction were analyzed. All patients were divided into two groups. 35 eases ( Group 1 ) were underwent ultrasound-directed follicular aspirations and 23 cases( Group 2 ) were give gynecological examination with mechanical extruding. Results All 35 cases in group I were successful in ovulation induction. Eleven pregnancies and one moderate ovarian hyperstimulation syndrome (OHSS) occurred in group 1, resulting in a pregnancy rate of 31.4 %, and an incidence of moderate OHSS 2. 9 %. Two pregnancies and 11 ovulation induction developed in group 2 with 3 moderate OHSS, resulting in a pregnancy rate of 8.7 %, an incidence of ovulation 47. 8 %, and an incidence of moderate OHSS 13.0 %. No severe cases of OHSS were noted in either of the two groups. Comparison of the therapeutic effects between the two groups showed significant difference ( P 〈 0. 001 ), with higher incidence of ovulation in group 1 than in group 2. No significant differences were noted in pregnancy rate and incidence of moderate OHSS ( P 〉 0. 05 ). Conclusion The therapy of ultrasound-directed follicular aspirations is effective in treating LUFS with PCOS in ovulation induction.
