摘要
目的探讨居家腹膜透析(PD)患者的社区管理对策。方法选取2006—2009年在我院什刹海社区卫生服务中心治疗的40例腹膜透析患者,建立患者病情档案以为社区护理提供基础资料,定期进行社区管理干预,包括为患者提供心理咨询、健康教育、护理指导等。对社区管理前后患者的水肿情况和其他各观察指标情况进行比较。结果腹膜透析患者社区管理前后水肿情况比较,差异有统计学意义(P<0.01)。腹膜透析患者社区管理前后外出口感染、操作熟练、感觉舒适、参加社会活动等指标比较,差异均有统计学意义(P<0.05)。结论社区管理是居家腹膜透析患者长期健康生存的重要环节,对患者的生活质量及存活年限有很大影响。
Objective To probe into the community-based management measures for patients with peritoneal dialysis （PD） at home.Methods Totally 40 PD patients treated in the Shenshahai community health service center of our hospital from 2006 to 2009 were recruited.Their records were set up as the basic data fro the community-based nursing intervention,which was regularly conducted,including psychological counseling,health education,and nursing guidance.The edema conditions and other indexes of the patients were compared before and after the treatment.Results The edema condition of the patients was obviously improved after the community-based management （P0.01）.And the differences between pre-management and post-management showed statistically significance in infection of outer opening,proficient operation,comfortable sensation,and participation in community activities （P0.05）.Conclusion Community-based management is a important link for a long-term health survival of patients with peritoneal dialysis at home,with a great impact to the patient′s quality of life and length of survival.
