摘要
目的探讨自发性气胸的电视胸腔镜手术治疗的方法及临床评价。方法对57例胸腔镜手术治疗的自发性气胸进行回顾性分析。结果 57例均存在肺尖病灶,其中上肺单发病灶36例,多发病灶18例,多叶病灶3例。手术时间平均55分钟,术后胸腔引流管平均引流量180mL,拔胸腔引流管时间平均为3.8天,术后住院时间平均为8天,全组治愈出院。术后病理检查结果:大泡型肺气肿37例,囊泡型肺气肿18例,支气管型肺囊肿2例。随诊2个月至62个月,有1例复发,保守治疗后治愈。结论自发性气胸首选电视胸腔镜手术治疗。
Objective To investigate the therapeutic method and clinical efficacies on pneumothorax with video assisted thoracoscopy. Methods From November 2000 to November 2009, 57 cases with spontaneous pneumothorax received video assisted thoracoscopy operative treatment were analyzed retrospectively.Results The lesions of fifty seven cases located in the apex of lung. Among them, upper pulmonary single lesion, multiple lesions and multiple lobe lesions were in 36,18 and 3 cases respectively.Average operative time was 55 minutes.Average postoperative thoracic drainaged volume was in 180 ml.All cases were cured to discharge from hospital.Postoperative pathological examined resuhs：bullous type pulmonary emphysema in 37 cases,vesicular type pulmonary emphysema in 18 cases and bronchial type pulmonary cyst in 2 cases.Follow-up ranged from 2-62 months. Follow- up was got in 57 cases Only one case was recurrence in all cases,which this case was cured via conservative treatment. Conclusion Video assisted thoracoscopic surgery is a first choice for spontaneous pneumothorax.
