摘要
目的探讨常年性变应性鼻炎(AR)患者中户尘螨的地位及其与血清嗜酸细胞阳离子蛋白(eosinophil cationic protein,ECP)的关系。方法102例具有常年性AR临床表现的疑似患者行吸入性变应原过筛试验(Phadiatop)、血清特异性IgE(specificIgE,sIgE)及血清ECP检测,分为阳性组(Phadiatop与血清户尘螨sIgE均阳性),阳性对照组(Phadiatop阳性,血清户尘螨sIgE阴性),阴性组(Phadiatop与血清户尘螨sIgE均阴性)。结果阳性组45例(44.12%),阳性对照组7例(6.86%),阴性组48例(47.06%),其中2例Phadiatop阴性而血清户尘螨sIgE阳性者未入组。102例对象中,Phadiatop阳性者52例(50.98%),血清ECP阳性者66例(64.71%),两者有显著性差异(χ2=5.16,P<0.05);阳性组与阴性组血清ECP比较有显著性差异(P<0.01);阳性组血清户尘螨sIgE与ECP之间存在相关性(r=0.448,P<0.01)。结论在常年性AR患者中,户尘螨过敏者占大多数;对于具有常年性AR临床表现的疑似患者,血清ECP检测具有辅助诊断价值。
OBJECTIVE To investigate the position of Dermataphagoides pteronyssinus(Derp)sensitivity in perennial allergic rhinitis and its relationship with sereum eosinophil carionic protein(ECP). METHODS One hundred and two suspected perennial allergic rhinitis patients with typical clinical syndrome were enrolled in this study. Phadiatop test, specific IgE to Derp and serum ECP were sampled from all subjects. According to all test results, 102 subjects were divided into three groups: positive group(positive Phadiatop result, sIgE0.35 kIU/L), positive control group(positive Phadiatop result, sIgE≤0.35 kIU/L),negative group(negative Phadiatop result, sIgE≤0.35 kIU/L). RESULTS Based on all test results, two subjects were not enrolled in any group with negative Phadiatop test and positive sIgE results. There were 45 subjects in positive group(44.12%), 7 subjects in positive control group(6.86%)and 48 subjects in negative group(47.06%). The positive rate of Phadiatop test and serum ECP in all 102 subjects were significant different (χ2=5.16, P〈0.05). The ECP level of the Derp-sIgE in positive group was signi?cantly higher than that in positive control group(P〈0.01). There was a positive correlation between sIgE and serum ECP in positive group(r=0.448,P〈0.01). CONCLUSIONDerp sensitivity has a leading position in perennial allergic rhinitis, and serum ECP test is helpful in diagnosis of perennial allergic rhinitis.
