摘要
目的从相关细胞因子探讨宫内感染乙型肝炎病毒(HBV)导致乙肝疫苗免疫失败的机制。方法对HBV宫内感染免疫失败儿童13例(无反应组)、宫内感染免疫成功儿童13例(有反应组)和正常免疫儿童21例(正常组),测定血清可溶性白细胞介素2受体(sIL2R)和分离外周血单个核细胞(PBMC)给予非特异性刺激植物血凝素(PHA)及特异性刺激乙型肝炎表面抗原(HBsAg),进行体外细胞培养,用流式细胞仪检测PBMC白细胞介素受体(mIL2R,或称CD25),酶免夹心法检测PBMC受刺激后产生的IL2、sIL2R及IL10的含量。结果在PHA刺激下,三组对象PBMC表达mIL2R的比例相近,差异无显著性,(F=0107,P>005);而当HBsAg刺激下正常组与无反应组比较(t=1221,P<0001),有反应组与无反应组比较(t=743,P<0001),差异有极显著性。三组对象PBMC的mIL2受体表达水平无反应组最低,其次为有反应组,正常组最高,三组间差异有显著性。sIL2R水平相比无反应组低于有反应组和正常组,差异有极显著性。无反应组IL2产生低下,而IL10只见有反应组在外来刺激后升高?
Objective To study the immunoregulatory mechanisms of responsiveness and nonresponsiveness to hepatitis B vaccine (HBVac) by analyzing the changes of IL2/IL2R system and IL10 in children who had intrauterine hepatitis B virus (HBV) infection Methods Fortyseven subjects, including 13 children who had intrauterine HBV infection and were not responsive to HBVac (nonresponders), 13 children who had intrauterine HBV infection and were responsive to HBVac (responders) and 21 healthy children who were immunized against HBV (normal control)Peripheral blood mononuclear cells (PBMC) taken from the subjects were cultured in vitro and PBMC mIL2R was determined by using flow cytometry; IL2, sIL2R and IL10 contents were determined by using sandwich enzyme immunoassay Results After specific stimulation with HBsAg, the expression levels of mIL2R of the 3 groups showed significant difference, the lowest level was seen in nonresponder group and the highest in normal control group The level of sIL2R of the nonresponder group was lower than those of responders and normal control group; this difference was statistically highly significant The level of IL2 production of the nonresponder group was low While only the responder group had increased production of IL10 after exogenous stimulation Conclusion Low level of IL2 production and IL2R expression after the HBsAg stimulation may contribute to immunization failure in children with intrauterine HBV infection ;there may be an immune tolerance mechanism In responders who had intrauterine HBV infection, production of HBsAgspecific antibodies may be related to the elevated level of IL10
