支气管哮喘患儿Clara细胞分泌蛋白水平的变化及吸入糖皮质激素对其影响 被引量:8

摘要 目的探讨支气管哮喘(哮喘)患儿Clara细胞分泌蛋白(CCSP)水平变化及吸入糖皮质激素(ICS)对CCSP的影响。方法选取2004年7月-2007年12月在本院哮喘专科和儿科住院哮喘儿童60例。男39例,女21例;年龄3~12岁。选取同期本院门诊健康体检儿童30例为健康对照组。男20例,女10例;年龄3~12岁。二组儿童均晨起空腹抽取静脉血5mL,1500r·min-1离心10min分离血清。采用ELISA法检测2组儿童血清CCSP水平,哮喘儿童ICS治疗3个月、6个月、12个月分别抽血复查CCSP水平。结果 1.哮喘组患儿急性发作期、ICS治疗3个月、治疗6个月血清CCSP水平[(5.140±2.331)μg.L-1、(8.730±3.392)μg·L-1、(10.510±2.813)μg·L-1]均较健康对照组低[(13.230±4.010)μg·L-1,Pa<0.001];哮喘组患儿治疗12个月血清CCSP[(11.740±3.284)μg·L-1]与健康对照组儿童比较差异无统计学意义(P>0.05)。2.哮喘组患儿ICS治疗3个月、6个月、12个月血清CCSP水平均显著高于急性发作期(Pa<0.001)。3.哮喘患儿ICS治疗6个月、12个月CCSP水平均高于ICS治疗3个月(P<0.05,0.001),ICS治疗6个月与治疗12个月比较差异无统计学意义(P>0.05)。结论 CCSP在哮喘呼吸道炎症中起保护作用,糖皮质激素可促进CCSP分泌,糖皮质激素与CCSP在抗哮喘呼吸道炎症方面可能有协同作用。 Objective To explore the changes of Clara cell secretory protein(CCSP) in asthmatic children and the effects of inhaling glucocorticoid (ICS) on CCSP.Methods Sixty children with asthma were selected as asthma group(in which 39 cases were male and 21 cases were female,aged 3-12 years old) and 30 healthy children were selected as healthy control group(in which 20 cases were male and 10 cases were female,aged 3-12 years old).Venous blood samples were collected in asthma group and healthy control group in morning before breakfast,and then sera were obtained by centrifuge in speed of 1 500 r·min^-1 in 10 min.The dynamic levels of CCSP were measured in sera by enzyme-linked immunosorbent assay.Results 1.In asthmatic children,the CCSP levels in acute episode,3 months after ICS,6 months after ICS, and 12 months after ICS[(5.140±2.331)μg·L^-1,(8.730±3.392)μg·L^-1,(10.510±2.813)μg·L^-1]were all lower than that in healthy control group[(13.230±4.010)μg·L^-1](Pa0.001),12 months after ICS,the CCSP levels returned to common level[(11.740±3.284)μg·L^-1],and compared with the healthy control group,there was no significant difference (P〉0.05).2.Compared with acute episode,the patients who ICS for 3 months,6 months and 12 months had significantly higher levels of CCSP (Pa〈0.001).3.CCSP levels in asthmatic children who ICS for either 6 months or 12 months were significantly higher than that in children who just ICS for 3 months(P〈0.05,0.001,respectively),but there was no difference between the 2 groups who ICS for 6 months and 12 months(P〉0.05).Conclusions CCSP may play a protective role in the airway inflammation of asthma.Glucocorticoid may increase CCSP level in asthmatic children.Glucocorticoid and CCSP may cooperate in anti-inflammation in airway of asthmatic children.
