ALA-PDT治疗男性复发性尿道口尖锐湿疣的疗效观察 被引量:13

摘要 目的探讨5-氨基酮戊酸-光动力疗法(ALA-PDT)治疗男性复发性尿道口尖锐湿疣的临床疗效及不良反应。方法 73例男性复发性尿道口尖锐湿疣患者随机分为两组,分别给予ALA-PDT治疗或CO2激光烧灼法去除疣体,随访3月,观察疗效、复发率及不良反应。结果试验组患者经单次或再次ALA-PDT治疗后治愈率分别为65.12%,83.72%,均显著高于对照组的40.00%,63.33%。试验组术中不良反应以疼痛为主(48.84%),高于对照组(13.33%),术后不良反应仅见疼痛(32.56%),而对照组分别有疼痛、肿胀和局部感染发生(60.00%),前者总发生率显著低于后者(P<0.05)。结论 ALA-PDT疗法能有效降低男性复发性尿道口尖锐湿疣患者的复发率,与传统的CO2激光法相比,具有治愈率高、痛苦少、便于患者自我护理等优点。 Objective To explore whether 5-aminolevulinic acid photodymic therapy(ALA-PDT) can reduce the recurrence of eondyloma acuminatum on male Urethra. Methods Seventy-three male patients of recurrent urethral condyloma aeuminatum were randomly divided into two groups. The warts of trial group were exorcized by ALA-PDT therapy, while CO2 laser was traditionally used to the controls,with a follow-up of 3 months to evaluate the therapeutic effect, recurrence rate and untoward reaction. Results The recovery rate after the 1st and the 2nd therapy were 65.12% and 83.72% respectively in the trial group, which was significantly higher than the control(40.00%, 63.33% ). Pain was the predominant intraoperative untoward reaction with the incidence rate of 48.84% in the trial group, higher than that in the control ( 13.33% ). Meanwhile pain was the only postoperative untoward reaction in the trial group, which was statistically lower than the pain, swelling and infection in the control group (32.56% vs 60.00% ). Conclusion ALA-PDT therapy can reduce the recurrence of condyloma acuminatum on male Urethra, with the advantage of higher cure rate, lesser pains and more convenient to the self care than the traditional therapy of CO2 laser.
