血清PCT水平指导老年下呼吸道持续细菌定殖者抗生素治疗的价值 被引量:4

摘要 目的评价采用血清降钙素原(PCT)指导老年下呼吸道持续细菌定殖患者抗生素治疗的价值。方法 56例机械通气下呼吸道持续细菌定殖的患者随机分为PCT指导组和常规经验治疗组。根据血清PCT水平决定是否应用抗生素,PCT<0.25μg/L不主张应用,0.25μg/L≤PCT<0.5μg/L时根据临床情况和医生经验决定是否应用抗生素,PCT≥0.5μg/L时建议应用。结果 PCT指导组与常规经验治疗组相比抗生素暴露下降RR 0.67(95%Cl 0.47~0.95,P<0.05),PCT指导组抗生素应用减少29%,两组间临床结果无明显差异(P>0.05)。结论以生化标志物PCT水平指导抗生素治疗,可以减少老年下呼吸道持续细菌定殖患者的抗生素使用,参照PCT水平为基础的抗生素治疗方法在当今抗生素过度应用现状下有重要的临床和经济学意义。
