腮腺肿块的螺旋CT常规与灌注成像诊断 被引量:9

Conventional and Perfusion Spiral CT Imaging in Diagnosis of Parotid Mass

摘要 目的探讨螺旋CT常规及其灌注成像诊断腮腺良恶性肿块病变的价值。资料与方法对34例(39个病灶)进行常规CT扫描,其中21例(23个病灶)行CT灌注成像,对比分析良恶性肿块的影像表现。结果常规CT扫描示良性肿块29个,恶性10个,在肿块边缘、邻近组织侵犯、邻近周围是否有肿大淋巴结方面,良、恶性肿块之间差异有统计学意义(P<0.05);CT灌注扫描示良性肿块17个,恶性6个,灌注血流量(BF)、血容量(BV)、血管表面通透性(PS)在良、恶性肿块之间差异有统计学意义(P<0.05)。而常规CT和灌注CT扫描在病灶密度、囊变、最大径、强化值和灌注平均通过时间(MTT)方面差异均无统计学意义(P>0.05)。结论螺旋CT常规扫描结合灌注成像可提高诊断腮腺肿块的准确性。 Objective To investigate the value of conventional and perfusion spiral CT imaging in benign and malignant masses of the parotid. Materials and Methods 34 patients with 39 masses examined by routine spiral CT scan in the parotid,21 cases with 23 lesions were performed by CT perfusion imaging. The findings of benign and malignant masses were compared and analyzed. Results 29 benign masses and 10 malignant masses were found in 34 cases in conventional spiral CT scan. There were significant differences between these benign and malignant masses such as the edge of lesions,adjacent tissues offending,neighboring lymph nodes existed or not (P〈0.05).17 benign masses and 6 malignant masses were discovered in 21 cases in CT perfusion imaging. It had significant differences between the benign and malignant masses in perfusion parameter of BF、BV and PS (P〈0.05). However,the differences did not obviously significance in density,cystic degeneration,the biggest diameter,enrichment value,and perfusion parameter of MTT (P〈0.05). Conclusion The conventional spiral CT combine with CT perfusion imaging can improve the accuracy of diagnosis in the benign and malignant masses of the parotid.
