左右肘静脉注射对比剂对CT增强效果的影响 被引量:11

摘要 目的比较左或右不同肘静脉注射对比剂的CT增强效果,探讨两者之间差异的影响因素。资料与方法选择行胸部平扫加增强扫描患者共166例。按分层随机分组法,根据年龄分为青年组(<40岁)、中年组(40~60岁)、老年组(>60岁);再行左或右不同肘静脉注射部位的随机化分组。所有病例均采用多层螺旋CT,扫描范围从肺尖至肺底,注射对比剂后65s进行增强扫描。测量左侧头臂静脉横跨主动脉弓(或头臂动脉)与胸骨柄后缘之间的前后径,对比分析其径线大小与CT增强效果的关系。结果左侧头臂静脉横跨主动脉弓(或头臂动脉)与胸骨柄后缘之间的前后径在不同年龄组的比较中,青年组与老年组之间差异具有统计学意义,青年组与中年组及中年组与老年组之间差异均无统计学意义。该径线与左肘静脉注射对比剂后的图像增强效果存在关联性,径线大者增强效果好,差异具有统计学意义(P<0.05);左或右肘静脉注射与图像增强的效果存在关联性,右肘静脉注射者图像增强效果较好,差异具有统计学意义(P<0.05)。结论 CT增强扫描时,于右肘静脉注射,可避免部分患者因左头臂静脉受压影响图像增强的效果。 Objective To explore the relationship of right or left-side ulnar vein injection and quality of enhanced CT images. Materials and Methods 166 consecutive patients underwent thoracic CT contrast enhancement examination with multidetector CT. Scan delay time was 65 sec. According to stratified randomization,subjects were divided into three groups,with right or left vein injection. Anteroposterior dimension between posterior border of manubrium sterni and anterior border of aortic arch(or brachiocephalic artery) where left brachiocephalic vein acrossing was measured and analyzed with relation of image quality. Results Compared with different groups,anteroposterior dimension showed a statistical significant difference on〈40 years group and〉60 years groups,no statistical difference on 〈40 years group and 40-60 years group,40-60 years group and60 years group.This dimension had a relevance with quality of enhancement images in left ulnar vein injection(P〈0.05).Right-or left-side ulnar vein injection had a relevance with quality of enhancement images,right ulnar vein injection got better quality of enhancement images than left(P〈0.05). Conclusion CT contrast enhancement examination with right ulnar vein injection rather than left side may improve the quality of images by avoiding compression of left brachiocephalic vein in some patients.
