摘要
目的探索一种适用于大面积深度烧伤患者微粒皮手术失败后肉芽创面覆盖的新途径。方法取患者近亲属断层刃厚异体皮剪成0.3cm×2cm长方形,按0.5~0.8cm间距植于清创后的肉芽创面,再将自体刃厚皮剪成0.2cm×0.2cm正方形植于异体皮间距中,取皮面积可按患者供皮面积的大小而定,切取自体皮与异体皮的比例为1∶1.5~1∶2。结果用此方法修复16例大面积深度烧伤患者后期肉芽创面,植皮后自体皮及异体皮均成活良好,创面愈合。随访6个月~2年,愈合创面均无明显疤痕增生及异体皮排斥现象。结论亲属异体刃厚皮与自体刃厚皮混合移植为修复大面积深度烧伤患者后期肉芽创面开辟了一种新途径,且预后无明显疤痕增生,很大程度上节省了医疗费用,值得推广。
Objective To explore an effective method of covering the granulation wounds of large area deep burn patients who failed particulate skin grafting. Methods Related donor allo-skins (0.3 cm×2.0 cm in rectangle size) were transplanted in granulation wounds after debridement, by distance 0.5-0.8 em and separated by auto graft of stamp thin-thieked skin (0.2 cm ×0.2 cm in size). The grafting area ratio of allo-skin and auto-skin was 1 : 1.5-1 : 2. Results A good survival rate of both allo-skin and auto-skin was achieved in all granulation wounds of all sixteen severe burn patients by above-mentioned methods. Limited graft rejective reaction phenomenon and less cicatricial contracture were observed in healed burn wounds with six months to two years follow-up visits. Conclusion Intermingled transplantation of related donor ( homo- genic) skin and auto-skin may be a new way of skin transplantation with less cicatricial contracture and saving medical costs.
