盐酸戊乙奎醚预处理对心脏瓣膜置换术患者血浆心肌肌钙蛋白I和肌酸激酶同工酶MB水平的影响 被引量:4

摘要 目的:观察不同剂量盐酸戊乙奎醚(PHC)对心肌缺血-再灌注损伤的保护效应。方法:择期行心脏瓣膜置换术患者45例,随机分为对照组(C组)、PHC组1(P1组)和PHC组2(P2组),每组15例。P1和P2组于体外循环前10min经中心静脉分别给予PHC0.04和0.08mg/kg,C组给予等体积生理盐水。分别于体外循环前、主动脉开放即刻、主动脉开放后30min、4h和24h收集桡动脉血,检测血浆心肌肌钙蛋白I(cTn-I)和肌酸激酶同工酶MB(CK-MB)。结果:主动脉开放后,3组患者各时点cTn-I和CK-MB水平比较差异均有统计学意义(cTn-I:F时间=43.908,P<0.001;CK-MB:F时间=188.526,P<0.001),但P1组和P2组血浆cTn-I和CK-MB水平低于对照组,差异均有统计学意义(cTn-I:F组间=4.608,P=0.016;CK-MB:F组间=17.402,P<0.001)。结论:PHC预处理在一定程度上可以减轻心肌再灌注损伤;0.04和0.08mg/kgPHC所产生的心肌保护作用无差别。 Aim:To investigate the protective effects of different dose of penehyclidine hydrochloride(PHC) pretreatment on myocardium against ischemia-reperfusion injury induced by cardiopulmonary bypass(CPB).Methods:Forty-five patients undergoing valve replacement with CPB were randomly divided into control group (n=15),PHC group 1(n=15)and PHC group 2(n=15).Before CPB,PHC was injected into the central vein with the dose of 0.04 and 0.08 mg/kg in group P1 and group P2(both diluted to 10 mL saline),and saline 10 mL in group C,respectively.Blood samples were taken from radial artery for determination of plasma cardiac troponin I(cTn-I) and creatine kinase isoenzyme MB(CK-MB) concentration before CPB(T1),at the moment of aortic unclamping(T2),at 30 min,4 h and 24h after aortic unclamping(T3,T4,and T5).Results:cTn-I and CK-MB of both groups had no significant different before CPB.After aortic unclamping,cTn-I and CK-MB of both groups had significant differences among five time points(cTn-I:Ftime=43.908,P0.001;CK-MB:Ftime=188.526,P0.001).cTn-I and CK-MB in group P1 and group P2 were significantly lower than those in group C at the moment of aortic unclamping,at 30 min,4 h and 24 h after aortic unclamping(cTn-I:Fgroup=4.608,P=0.016;CK-MB:Fgroup=17.402,P0.001).Conclusion:PHC pretreatment could protect myocardium from ischemia-reperfusion injury induced by CBP to some extent,but the protective effect of the two dose has no different.
