摘要
目的研究甲状腺功能正常的丙型肝炎患者使用干扰素α治疗后甲状腺功能的变化。方法收集65例2008年1月—2010年3月我院门诊确诊丙型肝炎患者的资料。干扰素α治疗前以及治疗后每3个月监测甲状腺功能及甲状腺抗体水平。结果治疗前62例甲状腺功能正常,3例甲状腺功能减低(甲减)者给予左甲状腺素替代治疗。其中治疗前抗甲状腺球蛋白抗体高者32例(32/62,49.2%),抗甲状腺微粒体抗体高者19例(19/62,29.4%),促甲状腺激素受体抗体(TRAb)均为阴性。治疗1年期间54例甲状腺功能正常;6例出现甲状腺功能亢进,其中4例TRAb阴性者继续使用干扰素,另2例TRAb阳性者停用后随访甲状腺功能恢复正常;2例出现亚临床甲减,经左甲状腺激素替代治疗甲状腺功能正常。原有3例甲减替代治疗者均完成1年治疗。结论甲状腺功能正常的丙型肝炎患者使用干扰素治疗后甲状腺功能改变不是影响继续治疗的因素。
Objective To investigate the influence of interferon (IFN) α therapy on the changes in thyroid function in patients with hepatitis C, who have normal thyroid function pretherapy. Methods Clinical data of 65 outpatients with hepatitis C treated in our hospital from Jan. 2008 to Mar. 2010 were collected. Thyroid function and anti-thyroid antibodies were tested pretherapy and every 3 months post-therapy. Results Sixty-two patients had normal thyroid function pretherapy, and 3 patients with hypothyroidism received levothyroxine alternatively. Pretherapy 32 patients (49.2%) had elevated levels of antithyroglobulin antibody, 19 (29.4%) had elevated levels of antithyroid microsome and thyrotrophin receptor antibody (TRAb) was negative in all the patients. Thyroid function remained normal in 54 patients during 1 year of treatment. Six patients had hyperthyroidism, of whom 4 with TRAb negative continued IFN α therapy, and the other 2 with TRAb positive withdrew from therapy and thyroid function was found to be normal during follow-up. Two patients had hypothyroidism and were given levothyroxine alternatively to have thyroid function get normal. The 3 patients with hypothyroidism, who had received replacement therapy of levothyroxine, completed 1 year of IFN α treatment. Conclusion The changes in thyroid function when IFN α therapy is given in patients with hepatitis C, who have normal thyroid function pretherapy, have no effect on whether the therapy will be continued or not.
