CTA在指导脑膜瘤手术中的临床价值

摘要 目的:探讨CT血管造影(CTA)在指导脑膜瘤手术中的临床价值,为今后脑膜瘤的诊断提供临床依据。方法:回顾性分析我院2008年9月~2010年5月15例术前行头颅CTA检查诊断为脑膜瘤患者的临床资料,并经手术和病理证实。结果:15例脑膜瘤患者均获得肿瘤、血管以及颅骨的优质三位一体立体图像,CTA检查清楚地显示肿瘤血供,肿瘤与周围大血管和静脉窦的关系以及肿瘤对颅骨的侵蚀情况;15例脑膜瘤中,9例三维图像重建肿瘤轮廓清楚且血管重建图像肿瘤染色明显,血运较丰富,6例三维图像显示血运不丰富,均与术中所见一致。结论:CTA诊断脑膜瘤具有较好的三维显示能力,为脑膜瘤的外科手术治疗提供更科学更直观的影像资料增加手术的安全性,同时具有快速、经济、微创、信息量大、受检人员接受辐射小等优点,对指导脑膜瘤手术中具有重要的临床价值。 Objective:To discuss the clinical application value of CTA in guiding the patients undergoing meningioma operation and provide the clinical evidence for the future diagnosis of meningiomas.Methods:The clinical records of 15 patients with meningioma diagnosed by head CAT and confirmed by surgical-pathologic results from September 2008 to May 2010 were retrospectively analysed. Results: Tumor,vascular and skull obtained satisfactory Trinity of stereo images in 15 patients.CAT could clearly demonstrated the blood supply of tumor and the relationship between tumor and its surrounding great vessels and sinus as well as the erosive effect of tumor on the skull.Results of the 15 cases,three dimension images rebuilding showed the contour of tumor clearly, vascular reconstruction images presented obvious staining of tumor and showed abundant blood supply in 9 cases.Three dimension images showed poor blood supply in 6 cases.The operative findings were in concord with the CAT to depict meningioma.Conclusion:CAT has a better three dimension displaying ability to diagnosis of meningiomas,hence offerred scientific and intuitive images for surgical operation treatment and improved the safety of operation.CAT also has the advantanges of rapid testing,solvent economy, less painful,informative and less radiation,and it has very important clinical value in guiding surgical treatment of patients with meningioma.
