摘要
目的探讨血浆γ谷氨酰转肽酶(GGT)与血压水平的关系。方法采用横断面研究方法,从门诊及体检人群中随机抽查945例作为研究对象,通过问卷调查及体检获取一般资料,包括年龄、性别、病史、用药史、家族史等,测量其身高、体质量、血压水平,检测血浆GGT水平、血脂谱、肝功能、血尿酸、血肌酐及尿素氮等。根据美国预防、检测、评估和治疗高血压全国联合委员会第7次报告将入选对象分为3组:正常血压组(≤120/80mmHg,n=403)、高血压前期组(120~139/80~89mmHg,n=209)和高血压组(≥140/90mmHg,n=333)。结果在正常血压组、高血压前期组及高血压组GGT水平逐渐增高,且GGT水平在三组间差别具统计学差异(P<0.01);Pearson相关分析显示GGT水平与平均动脉压、收缩压、舒张压、体质量指数、腰围、总胆固醇、三酰甘油、低密度脂蛋白胆固醇、肌酐及尿酸呈正相关(均P<0.05),与高密度脂蛋白胆固醇呈负相关(P<0.01)。以GGT为自变量,分别以收缩压、舒张压及平均动脉压为因变量回归分析显示,GGT每增加1mmol/L,收缩压、舒张压及平均动脉压分别增加0.28、0.21及0.23mmHg。仅调整性别、年龄进行回归分析显示GGT与平均动脉压、收缩压及舒张压独立相关(均P<0.01),而在调整了年龄、性别、体质量指数、腰围、三酰甘油、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、血肌酐、血尿酸、尿素氮及空腹血糖后,GGT未独立影响平均动脉压、收缩压,但其是舒张压的独立影响因素(P<0.05)。结论 GGT水平与血压水平呈正相关,可能是舒张压的独立影响因素。
Objective To explore the relationship between plasma Gamma-glutamyltransferase(GGT) and blood pressure.Methods By cross-sectional analysis,945 subjects were enrolled and separated into 3 groups according to JNC 7 (the Report from the Seventh Joint National Committee for the prevention,detection,evaluation and treatment of high blood presure),including normal blood pressure group (≤120/80 mm Hg,n=403),prehypertension group(120-139/80-89 mm Hg,n=209) and hypertension group(≥140/90 mm Hg,n=333).The medical history,family history and physical situation were accessed by questionnaire and physical examination,and biomarkers were detected by auto-machine,including GGT,liver function,cholesterol,triglycerides(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),fasting plasma glucose(FPG),blood uric acid,creatinine and urea nitrogen.Results Plasma GGT increased with blood pressure,and the differences existing among the three groups were significant (all P0.01);Pearson correlation analysis showed that GGT was positively related with mean arterial pressure (MAP),systolic blood pressure (SBP),diastolic blood pressure(DBP),body mass index (BMI),waist circumference,total cholesterol,TG,LDL-C,creatinine and uric acid in both male and female subjects(all P0.05),but negatively related with HDL-C (P0.01).After adjusting for age and sex,multiple regression analysis revealed that GGT was an independent factor of MAP,SBP and DBP (all P0.01);However,after adjusting for age,sex,BMI,waist circumference,TG,HDL-C,LDL-C,blood uric acid,creatinine and urea nitrogen and FPG,multiple regression analysis revealed that GGT was an independent factor of DBP (P0.05).Conclusion Plasma GGT is positively related with blood pressure,and GGT may be an independent factor of DBP.
