摘要
目的探讨血压正常高值者中心动脉压及其反射波与动脉弹性的关系。方法选择正常血压者159例,血压正常高值者149例,高血压者153例,应用大动脉测量仪测量中心动脉压及其反射波;应用脉搏波传导速度测定仪测定颈桡动脉脉搏波传导速度(crPWV)。结果血压正常高值组中心动脉收缩压(CSP)、舒张压(CDP)、中心脉压(CPP)、平均收缩压(CMSP)、平均舒张压(CMDP)、收缩末压(CESP)、第一峰值血压与最小血压之差(P1Height)及crPWV值均高于正常血压组,低于高血压组(P<0.01或P<0.05)。多元线性回归分析显示,CDP、CMDP、CPP是crPWV的影响因素(β值分别为0.314,0.223,0.196;均P<0.05)。结论血压正常高值者中心动脉压及其反射波已经增高,动脉弹性已经减退,CDP和CPP是影响动脉弹性的主要因素。
Objective To investigate the relationship of central blood pressure and its reflected wave with arterial elasticity in prehypertensive patients.Methods According to their blood pressure level,461 individuals were divided into three groups.Namely,normotensive controls (n=159),prehypertensive participants (n=149),and hypertensive patients (n=153).Central blood pressure and its reflected wave were detected noninvasively with SphygmoCor (AtCor Medical,Australia) device,and carotid-radial pulse wave velocity (crPWV) was obtained by Complior device (Artech Medical,Pantin,France).Results Central systolic pressure (CSP),central diastolic pressure (CDP),central pulse pressure (CPP),central mean systolic pressure(CMSP),central mean diastolic pressure(CMDP),central end systolic pressure(CESP),P1 Height and the values of crPWV were higher in prehypertensives than those in normotensives (P0.01 or P0.05),but lower than those in hypertensives (all P0.01).Multiple linear regression analysis showed that CDP,CMDP and CPP were risk factors for crPWV (β=0.314,0.223,0.196,all P0.05).Conclusion Central aortic pressure and its reflected wave in prehypertensive patients have already increased,while their arterial elasticity has decreased.CDP and CPP are the important influencing factors of arterial elasticity.
