再探糖化血红蛋白、尿微量白蛋白与早期糖尿病肾病的关系

摘要 目的探讨糖化血红蛋白(HbA1c)、尿微量白蛋白(U-mAlb)与早期糖尿病肾病的关系及临床应用价值。方法对508例已确诊糖尿病患者进行HbA1c、U-mAlb的测定,以HbA1c≤7%(A)、7.1%~9%(B)、9.1%~11%(C)、〉11%(D)为标准分组,比较不同HbA1c分组的U-mAlb异常检出结果。结果 A组与B、C、D各组比较有显著性差异(p〈0.001);B、C、D三组间无差异(p〉0.05)。结论 HbA1c与U-mAlb联合检测是发现和监控发生早期糖尿病肾病的有效手段。HbA1c控制良好者会大大降低糖尿病患者发生糖尿病肾病的危险性。 Objective Restudy the relationship between glycosylated haemoglobin(HbA1c),micro U-albumin(U-mAlb) and early diabetes nephrosis and investigate its clinical significance.Methods HbA1c and mAlb were detected in 508 patients who have already diagnosed as diabetes.4 groups were divided by the standard of HbA1c≤7%(A);7.1%~9%(B);,9.1%~11%(C);and 11%(D).Then the value of mAlb was compared among four groups.Results Group A showed significant level(P0.001)compared with other groups;there is no difference between group B,C and D(P0.05).Conclusion Joint detection of HbA1c and U-mAlb is effective way to diagnose and monitoring the diabetes nephrosis.Controlling the level of HbA1c better will greatly decrease the chance of diabetes nephrosis.
